The Family Outreach Worker plays a vital role in connecting individuals and communities with essential resources and services. The Family Outreach Worker's primary role is to proved direct assistance in the implementation of program & service needs on behalf of the Program Manager and CHYTC.
Responsibilities
Provides direct support to clients through one-to-one or in a group setting to clients in fulfilling program objectives and goals.
Provides advocacy for clients through resourceful support to meet and achieve program delivery.
Provide direct mentorship for clients promoting self-esteem, awareness, and healthy choices.
Ensuring compliance with applicable laws, regulations, and licensing requirements to promote health and safety.
Assists in implementing and monitoring quality improvement initiatives to enhance the overall service delivery outcomes.
Works closely with the CHYTC Program manager and staff in meeting client goals and program requirements. Proving status updates to the Program Manager, highlighting achievements, challenges , and action plans.
Ensures compliance with CHYTC operational policies and standards.
Maintain accurate records of outreach activities, participant interactions, and feedback for continuous improvement.
Participate in training sessions and workshops to stay informed about best practices in outreach and service delivery.
Requirements
Certificate, Diploma, or Degree in the Human Services related discipline.
1 or more years related experience.
Clear Criminal Records Check & Clear on Child Welfare Involvement Check.
First Aid Certification.
Open availability to provide services to youth on weekdays, evenings, and weekends to accommodate to needs of clients.
Ability to work with youth and families from diverse communities.
Knowledge and or understanding of First Nations Culture and Traditions.
This role is essential for fostering connections between individuals in need and the resources that can empower them toward greater independence and quality of life.

Pay: $25.00-$30.00 per hour
