Job Description

Compassionate Neighbours is a registered charity serving seniors and caregivers in Chilliwack and the surrounding area through one-to-one and group activities and programs. Thanks to the Province of BC and United Way, we have received funding to expand Caregiver support through a Family & Friend Caregiver Support Program. This role will beunder the direction of the Executive Director or a delegated Supervisor, ensuring grant deliverables.

PROGRAM GOALS:



To establish a space that provides one-to-one support by telephone or in person To host a weekly Caregiver Support Group with a contracted facilitator To increase caregivers' skills, knowledge, and confidence to care. To increase caregivers' sense of social connectedness To improve caregivers' connection with health care teams as a partner in care To improve caregivers' regular use of formal and informal respite to reduce burnout To increase support to First Nations' Elders' and their caregivers as needed

GENERAL RESPONSIBILITIES:



Coordination and Delivery of Programs:



In accordance with Compassionate Neighbours' Policies and Procedures and the Handbook for Family and Friend Caregiver Support Coordinators, develop, facilitate and complete tasks necessary to deliver the following programs:

One to One Support and Referrals Caregiver Support Group(s) Healthcare System Navigation Connection to Community Services Knowledge and Skill Enhancement Holiday Meals (Easter, Thanksgiving, Christmas) Outreach to Elders

Administration:



Tracking of data Requests for support Referrals received Attendance Ensure venues are suitable for participant needs Ensure supplies and documents (calendars, program info, resource information, Certificate of Insurance) are readied for programs and events Identify number of volunteers required for programs and liaise with Volunteer Coordinator to ensure efficiency, program success and safety Develop and maintain resource list for caregiver support Develop survey or similar opportunity for participant, speaker, facilitator feedback for programs Develop Quality of Life survey as measurement tool for program impact Submit cheque requisition to Program Administrator for honoraria, venue or catering costs, prior to events when necessary Submit expense reports to the Program Administrator Prepare monthly report to the Executive Director including statistics and anecdotes

Community Engagement:



Develop community connections where appropriate to share Compassionate Neighbours programming and to identify gaps in services Identify programs currently in place that would benefit seniors and caregivers Identify program capacity and need for additional days/times/locations Research new opportunities for collaboration with social service agencies, health services, health care providers and others who support seniors and their caregivers. Liaise with local First Nations' Bands to identify needs and opportunities within their community for Elders and their Caregivers Identify spaces suitable for events, programs and meetings

QUALIFICATIONS:



Preference is for candidates with a Diploma in Human Services or Certificate as Community Support Worker or Health Care Worker A minimum of one year of experience working or volunteering in the Social Services Sector or with Indigenous communities in a culturally safe way A combination of lived experience, work and education will be considered Knowledge of the history of Indigenous peoples in Canada and the impacts of colonization is an asset Excellent verbal and written communication in English Excellent organizational, interpersonal, group team building and facilitation skill Experience with financial reconciliation and budgets Adherence to the Society's hiring and screening protocols including reference checks and a Criminal Record Check for the Vulnerable Sector Possess a Valid Class 5 Driver's Licence Agreement of Compassionate Neighbours' (Compassionate Neighbourhood Health Partners Society) Vision, Mission and Core Values (see About tab) Eligible to work in Canada at the time the application is submitted
