Working in collaboration with other Alzheimer Society HPE team members, the Education & Support Coordinator will coordinate and/or provide the delivery of client dementia education programs, information, consultation, support and referral of community resources in the manner most appropriate, determined by needs and availability of services, to those in need of those services. The ASHPE provides services throughout Hastings & Prince Edward Counties and the Municipality of Brighton. The successful candidate will work both within the community and out of the Belleville office. Their main geographical areas of focus will include (but is not limited to) Centre Hastings.

Primary Duties and

Responsibilities:



Education Services Delivery

To identify community educational needs, and act as an education resource for the general public Design, deliver and evaluate educational and training events for clients (persons with dementia and their caregivers) and some professional groups and service providers in a range of settings related to Alzheimer disease, related dementia and caregiving

Support Services Delivery

work in collaboration and partnership with Alzheimer Society First Link Care Navigator to ensure delivery of client services, support and follow-up*
Identify support needs in the community Consult with persons with dementia and/or their care partners in the most appropriate manner and location Consult with persons with dementia and/or their care partners who are in need of help to find solutions, make choices, explore options, identify needs, locate information, and promote strategies to address issues presented as a result of dementia (these activities may extend beyond regular operational hours). Provide referrals to the most appropriate resources for the clients' needs, whilst ensuring that referrals receive timely and appropriate service Co-ordinate and/or facilitate support groups, ensuring a safe and comfortable outcome Establish and maintain relationships with community partners/agencies, stakeholders and referral sources

Database and Information Management

Maintain up-to-date statistics, and contribute to monthly reports on education and support activities Using data, and in consultation with colleagues and Program Manager, evaluate effectiveness of education and support programs, and collaboratively find solutions to concerns and challenges pertaining to education and support services

Community Relations/Advocacy

Maintain professional, effective communication with individuals at all organizational levels including clients, caregivers, volunteers, service providers and other community organizations Initiate opportunities to communicate Alzheimer Society mission, programs and support to local community and other social service agencies

Technology - experience with Microsoft Office programs (Word, Excel & Power Point), AlayaCare database.

Physical Demands:

Performance of general office duties

Significant periods sitting at the computer, but with the opportunity to move away from the work station

Sensory attention is required for the majority of the work day (looking at computer screen, reading documents, etc.)

Education/Experience:



SSW College Diploma, or University degree with 3 years experience working with Persons living with dementia/care partners. Other Post-secondary degree or diploma in a health disciplines may be considered given appropriate combination of education and experience.

Successful Candidate will need: Police check at vulnerable sector; Access to a vehicle; Valid driver's license and insurance; Ability to work some evenings and weekends as required.

35 hours per week
