Job Summary


We are seeking a compassionate and energetic Disability Support Worker to empower individuals with disabilities to live fulfilling, independent lives. In this vital role, you will provide personalized assistance, promote safety, and foster a supportive environment that encourages growth and well-being. Responsibilities



Deliver direct support to individuals with disabilities by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility. Monitor patients' vital signs and overall health status, promptly reporting any concerns to healthcare professionals. Administer medications accurately following care plans and ensure compliance with HIPAA regulations to protect client confidentiality. Support clients during meal preparation, cooking, and nutrition planning to promote healthy eating habits. Assist with mobility using equipment such as Hoyer lifts or resident's personal assistive devices while ensuring safety through proper techniques. Implement individualized care plans tailored to each client's needs, including dementia care, Alzheimer's care, autism support, or memory care. Maintain a safe environment by performing routine cleaning tasks in accordance with health standards and organizational policies. Observe patient behavior closely for signs of distress or changes in condition; document observations meticulously for ongoing care adjustments. Engage clients in social activities and community outings to foster socialization and community integration. Collaborate with healthcare teams, social workers, and family members to coordinate comprehensive care strategies. Respond effectively during emergencies by executing CPR or first aid procedures when necessary. Support hospice or end-of-life care efforts with sensitivity and professionalism. Use patient monitoring tools and recordkeeping systems accurately to track progress and ensure compliance with all regulatory standards.

Qualifications



Previous experience working in assisted living facilities, nursing homes, group homes, or home & community care environments is highly preferred. Certification in First Aid and CPR is required; additional certifications such as HIPAA training are advantageous. Demonstrated ability to work with people with developmental disabilities, dementia, Alzheimer's disease, autism spectrum disorder or other special needs populations. Knowledge of medication administration procedures and safety protocols related to patient care. Experience operating patient lifting equipment such as Hoyer lifts; ability to perform heavy lifting safely when required. Strong observational skills for patient monitoring and patient care documentation. Excellent communication skills for effective collaboration with team members and families. Ability to work flexible hours including evenings, weekends, or overnight shifts as needed. Compassionate attitude combined with patience and resilience in challenging situations.
