We are seeking a compassionate, energetic, and dedicated Disability Support Worker to join our team. Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor patients' vital signs and overall health status regularly to ensure safety and well-being
Administer medication accurately following care plans while adhering to HIPAA regulations for confidentiality
Support clients with meal preparation, cooking nutritious meals tailored to dietary needs, and maintaining cleanliness in living spaces
Use specialized equipment such as Hoyer lifts or other assistive devices to safely transfer residents with mobility challenges
Conduct patient observation and monitoring to detect changes in condition or behavior, especially for individuals with dementia or autism
Implement individualized care plans designed by healthcare professionals to meet each client's unique needs
Engage in patient care activities within assisted living facilities, nursing homes, or home & community care settings
Maintain accurate documentation of care provided, including medication administration, vital signs, and incident reports
Support clients during social activities and facilitate community integration to promote social well-being
Uphold safety standards by practicing first aid and CPR when necessary; respond effectively during emergencies or incidents
Skills
Proven experience working with people with developmental disabilities, autism, dementia (including Alzheimer's), or in memory care environments
Strong knowledge of patient monitoring techniques and vital sign measurement
Familiarity with care procedures such as assisting with ADLs, medication administration, and developing/implementing care plans
Ability to perform heavy lifting safely using equipment like Hoyer lifts; physically capable of supporting residents' mobility needs
Excellent communication skills for working collaboratively with clients, families, healthcare teams, and social workers
Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times
Experience in home health or hospice care is advantageous but not required; a background in senior or long-term care is highly valued
Ability to handle multiple responsibilities efficiently while maintaining a positive attitude in dynamic environments
Valid certifications such as CPR and First Aid are essential; additional certifications in nursing assistance or caregiving are preferred
