Provides leadership to the resident care team in all aspects of resident care within established legislative standards and professional standards in Long Term Care, Assisted Living and Independent Living.
Effectively manages human, material, and fiscal resources.
In collaboration with the resident care team develops resident focused programs, policies, and procedures.
Monitors the quality of care and services delivered to clients through effective quality improvement activities.
REPORTING RELATIONSHIP:
Reports to the Director of Long-Term Care Services
BASIC QUALITIFICATIONS FOR THIS POSITION INCLUDE:
Current RN certificate of competency from the College of Nurses of Ontario
Comprehensive knowledge of evidence based clinical practices
Comprehensive knowledge of long-term care concepts and philosophies
Comprehensive knowledge of Community Nursing concepts and philosophies
Excellent interpersonal and communication skills
Policy and program development
Must have a minimum of one (1) year of experience working as a RN in the long-term care sector
Must have a minimum of three (3) years of experience working as a Registered Nurse in a managerial or supervisory capacity in a health care setting.
Or
must have held a DNS position at a different LTCH without meeting the updated qualifications set out in the Regulation, on the condition that the person has had (3) years full-time experience in the position during the (5) years immediately before being hired at the different LTC home or the equivalent part-time experience in the position during the (7) years immediately before being hired at the different LTCH.
Knowledge of RAI-MDS - AIS certified, if not must be completed successfully within one (1) year of being hired.
Leadership
Provides direction to the nursing team in all aspects of resident care
Interprets nursing philosophy, and in collaboration with the nursing team establishes and implements goals and objectives.
Promotes the Person Directed Care philosophy
Holds meeting with Director of Long -Term Care Services on a regular basis
Introduces new care programs and services based on best practices
Interprets FLTCHA and Regulation 79/10 to the nursing team and monitors compliance to standards through continuous quality improvement activities
Demonstrates the ability to effectively communicate with staff, residents and families
Provides leadership and acts as a role model when dealing with staff, resident, and family conflicts
Acts as a mentor for Program Leads in the areas of personal and professional growth
Develops policies and procedures consistent with resident care practices, CNO, FLTCHA and Regulation, and other applicable legislation
Provides direction and support to Program Leads in each program area (i.e.: Skin Care and Wound Management, Falls Prevention, Incontinence Management, Infection Prevention and Control, etc.)
Promotes and fosters an environment where staff feel valued and involves staff at all levels with decision making
Along with the Nursing Unit Clerk approves resident applications, and assesses level of care requirements
Demonstrates sound knowledge of the resident bill of rights and monitors care to ensure rights of residents are always respected and promoted
Initiates and/or directs timely investigations of serious Residents issues including allegations of resident abuse
Human Resources
Recruits, hires, and discharges nursing staff
Adheres to human resources policies when recruiting and hiring new employees
Reviews new employee files to ensure the orientation is completed, and all the documentation is on file
Adheres to the collective agreement when developing and implementing human resources policies
Has a good understanding of staffing patterns, and scheduling activities
Manages staff absenteeism with the implementation of an effective attendance management program
Monitors payroll activities to ensure that wage grids, benefits, statutory holidays, vacations, lieu days are applies as per the collective agreements
Conducts performance management for the nursing department.
Counsels' staff in areas of non-compliance, inappropriate and unprofessional behaviour. Utilizes the progressive disciplining process when disciplining employees
Addresses all issues with staff in a professional manner
In collaboration with the Executive Director (ED), and the Quality of Worklife Team develops and implements recognition events
Refers employees to Employee Assistance Program (EAP) as needed
Provides direction to Staff Development Coordinator and monitors staff education activities to ensure all the mandatory in-services are covered
Demonstrates sound knowledge of policies contained in the employee handbook and full comprehension of the Resident abuse policy
Supports the RAI Coordinator in his/her role
Communication
Effectively communicates with staff at all levels, residents, families, other departments, and external stakeholders
Holds nursing meetings on a regular basis to introduce new programs, policies, quality improvements, new initiatives, changes in practices and policies
Able to diffuse conflicts effectively and is solution oriented
Quality/Risk Management
Assists in the co-ordination of the Quality Improvement program and follows the Residents First Initiatives with HQO. In collaboration with the QI Lead and the Resident Care and Managing Medications Team and Quality Team develops, tracks, and evaluates quality and risk indicators
Reviews trends quality improvement activities and takes the lead in changing practices, introducing new policies, programs, and services based on the identified trends and analysis
Conducts regular clinical chart audits to ensure compliance with documentation standards, internal policies and procedures, and FLTCHA and regulation
Collates and analyses the resident care section of the annual report
Assist with the evaluation of all resident programs and services for relevance, quality, and effectiveness
Identifies potential liability issues and takes action to minimize risk
Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported.
Required to participate in the improvement of the indicators and achieve satisfactory results.
Safety
Adheres to Occupational Health and Safety practices
Demonstrates good knowledge of emergency and fire plans through fire drills, and mock disasters
Promotes a safe working environment
Monitors resident safety and ensures that residents that are potential elopers, wear security bands
Reports to the MOHLTC all critical incidents as stipulated in the FLTCHA and Regulation 79/10
Investigates and reports employee work related accidents to the Occupational Health and Safety committee
Manages WSIB claims
Committee/team involvement
Chairs the Registered Staff meetings
Attends Registered staff meetings regularly
A member on the Leadership team, Quality Team, Resident Care and Management Medications team, Professional Advisory Committee (PAC), Labour Management meetings
Attends other committees/teams meetings as requested or required
Professionalism/Work ethic
Always conducts herself in a professional manner with dealing with staff, families, residents, and external clients
Represents the Home in a professional manner when attending outside seminars, committees, and meetings with outside agencies.
Adheres to the CNO standards of practice
Demonstrates honesty, and integrity in all activities
Completes all assigned training required as per the FLTCHA and Regulation 79/10.
