Reporting to the Program Manager of Ambulatory Care and in collaboration with the Medical Director for Endocrine and Diabetes, the Diabetes Nurse Educator provides and coordinates nursing practice activities and nursing practice education related to the care of patients with diabetes on inpatient units and ambulatory care settings. The Diabetes Nurse Educator acts as an expert clinical resource to nursing and other health care professionals within BCCH and throughout the province by teaching nursing practice related to Diabetes care to nursing staff, other health professionals and patients and their families.The Diabetes Nurse Educator is responsible for assigning direct care responsibilities to other nursing staff in accordance with established policies, standards, procedures and philosophy at British Columbia Children\xe2\x80\x99s Hospital. The Diabetes Nurse Educator is responsible for the planning, delivery, and coordination of education sessions and learning materials for nursing and other health care professionals for the Diabetes program and acts as a patient advocate, care facilitator and clinical nursing resource for diabetes patients and their families.What you\xe2\x80\x99ll do

  • Oversees the delivery of care for the diabetes patient population followed at British Columbian Children\xe2\x80\x99s Hospital by coordinating patient care delivery in other centers in British Columbia, ensuring provision of direct patient care for patients, and making adjustments in patient schedules and dosage regimens with accountability for follow-up and patient outcomes.
  • Initiates and coordinates assessment and post-assessment referrals; responds to patient, family and health care member\xe2\x80\x99s enquiries; and liaises with other support and diagnostic health services in coordinating patient referrals.
  • Provides diabetes care nursing education for nursing staff, students, and healthcare professionals by developing, evaluating and revising educational materials to enhance patient, family and health care professional\xe2\x80\x99s knowledge of diabetes management. Presents education sessions and rounds and evaluates learning needs of nursing staff and other health professionals and ensures materials are learned correctly.
  • Educates, counsels, advises and provides clinical expertise to health professionals and patients and families in the initiation, management and adjustment of insulin within CRNBC\xe2\x80\x99s nursing scope of practice and with C&W / BCCH certification for insulin dose adjustment and according to CDA Clinical Practice Guidelines and Standards for Diabetes Education.
  • Participates in orientation and continuing education of nursing staff and students by role modeling, preceptoring and participation in formal teaching. Provides education sessions to nursing staff and physicians through lecture and demonstration and follows up with to ensure that materials have been learned correctly.
  • Uses the nursing process and specialized diabetes nursing knowledge to provide primary nursing care and /or case management for patients/families. Teaches patients and families about diabetes in inpatient and ambulatory settings and provides ongoing follow-up education for families on an out patient basis. This may include telephone contact, individual appointments, regular workshops and training on diabetes technology (i.e. sensors and pumps).
  • Supports patients and their families with the use of diabetes technology (i.e. insulin pumps, sensors) by providing education, troubleshooting and making adjustments to dosages and/or settings.
  • Maintains current knowledge of new diabetes products and technologies for diabetes management; for example insulin pump therapy, continuous glucose monitoring and new insulin products.
  • Consults and collaborates with a multidisciplinary team and other health care professionals to coordinate referrals to other health care services, agencies and community supports.
  • Collects, documents, and maintains patient health care information regarding nursing observations, interventions, teaching and evaluation of nursing care.
  • Provides input to the Program Manager, and Endocrinology and Diabetes Medical Director regarding policies, procedures, and operational requirements by observing service needs and providing spoken or written suggestions which address patient and family needs and promote effective use of resources.
  • Serves as an expert clinical resource and provides education for health care professionals in the community and non-tertiary settings throughout the province of BC and Yukon territories by facilitating materials and courses, seminars, telephone consultation and mentoring to increase the health care professional\xe2\x80\x99s knowledge and understanding of diabetes in children, youth and families.
  • Coordinates diabetes care outside of BC Children\xe2\x80\x99s Hospital (i.e. outreach clinics, telehealth clinics) and acts as a community outreach resource province wide.
  • In collaboration with the care team, develops resources and strategies as required for patients and families to increase access to clinically relevant diabetes education and care in British Columbia.
  • Collaborates in the planning of patient care with nursing staff, medical staff and other health care professionals to plan, implement and evaluate nursing care for the patient and families in the community setting.
  • Provides consultation and education to Nursing Support Services to direct appropriate resources and support for children, youth and their families with diabetes in day care settings, school programs and the community.
  • Participates in the conduct, use and / or dissemination of nursing research and quality improvement activities in collaboration with or Nursing Practice Leaders; identifies issues and questions requiring research to advance evidenced based practice for diabetes.
What you bringQualifications:
  • Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nursing Professionals (BCCNP).
  • Certified Diabetes Educator (CDE), IDA Certification or equivalent combination of education, training and experience.
  • Three years recent related experience in ambulatory diabetes education and management.
  • CPR Training: Basic Cardiac Life Support (BCLS) \xe2\x80\x93 level 1.
