Diabetes Education Coordinator - Inpatient and Emergency Department - Permanent Full-Time
The Diabetes Nurse Educator will be responsible for working within a primary health care framework and a self-management model of service delivery. The successful candidate will provide clinical nursing-based care, education, and consultation to diabetes clients and their families/caregivers within the Diabetes Education Program and for supporting virtual patient consultations via Ontario Telemedicine Network (OTN). Additionally, the successful candidate will support development of clinical nursing skills amongst nursing staff within the Emergency Department and Inpatient [ST1] unit. The educator will model quality patient care and professionalism. They will assist in the onboarding of new nurses to develop the knowledge and skills required for practice within the clinical environment and will provide ongoing coaching and support to enhance the learning experience. The educator may have other duties assigned to them by the VP Patient Care and CNE to assist with clinical education of staff.
Primary Duties
Provides professional nursing care and education to clients, families, and caregiver participants of the Diabetes Education Program.
Triages and schedules Diabetes Education consultation requests and coordinates group programs
Develops, implements, and evaluates teaching programs, services, and follow-up in accordance with clinical practice and best practice guidelines for the prevention and management of diabetes and diabetes-related outcomes.
Coordinates and schedules appointments for specialist consultations via OTN and conducts nursing assessments of patients when needed.
Exercises clinical judgement in meeting patient needs.
Uses a variety of adult learning strategies and evaluation techniques to deliver education to patients and nursing staff in alignment with desired learning outcomes.
Reviews and assesses practice changes related to new products or equipment back orders.
Education & Experience
Bachelors Degree in Nursing (BScN).
Registration, in good standing, with the College of Nurses of Ontario.
Certification as "Certified Diabetes Educator" or intent to obtain certification within a prescribed time frame
Minimum of 5 years' experience working within an acute care environment.
Knowledge of Gentle Persuasive Approaches and Non-Violent Crisis Intervention an asset.
Knowledge of CNO standards and current best practice guidelines.
Proficient in documentation within Epic EMR system an asset.
Theoretical knowledge and practice experience in adult education, including educational program planning, development, and evaluation an asset
Skills & Abilities
Demonstrated ability to provide excellent customer service when dealing with patients, volunteers and other members of the community and public
Comprehensive knowledge of diabetes management, including insulin therapy, oral medications, glucose monitoring and complication prevention
Ability to conduct individualized assessments and develop evidence-based care plans, while building rapport with patients, families and caregivers
Excellent verbal and written communication skills for patient education, documentation and interdisciplinary collaboration
Strong computer skills and ability to work within Electronic Medical Records
Demonstrated skills in interpreting lab value, glucose logs and diagnostic information
Proficiency in teaching carbohydrate counting, nutrition basics and lifestyle modification
Familiarity with diabetes technologies including, but not limited to, glucometers, Continuous Glucose Monitors
Understanding of chronic disease management frameworks and patient-centered care models
Demonstrated patterns of consistent attendance and capability of maintaining that same standard.
Demonstrated commitment to the CMH core values
Compensation Disclosure: $45.55 to $56.77 per hour
