Diabetes Educator, Registered Dietitian or Registered Nurse
(24 hours/week, Contract to August 2026)

What We are Looking For:
We are seeking a
Diabetes Educator, RN or RD
to join our
Diabetes Education Team
in a
part-time role
. The successful candidate will provide
comprehensive diabetes education programs and individualized counseling
for adults living with
Type 2 diabetes
, both one-on-one and in group settings. Experience supporting clients with
Gestational Diabetes Mellitus (GDM)
and
Type 1 diabetes
is not an expectation but would be considered an asset. This position involves delivering education and support
both on-site and through community outreach settings,
contributing to improved self-management and health outcomes.
A successful candidate in this role would have
Baccalaureate preparation in Food and Nutrition or Nursing.
Registered Dietitian with the College of Dietitians of Ontario , or Registered Nurse with the College of Nurses of Ontario, in good standing
Certification as a Diabetes Educator (CDE) preferred
Experience counselling patients in diabetes clinic incorporating various behaviour change models, adult learning principles or techniques for chronic disease management preferred.
Experience with insulin therapy; initiation and adjustment.
Experience in providing education to youth and adults with Prediabetes, Type 2 and/or gestational diabetes
Comprehensive knowledge base in diabetes management is required
Familiarity of insulin pump therapy, or certification in pump training considered an asset.
Demonstrated ability to work autonomously and as a member of an interdisciplinary team.
Critical thinking skills
Comfortable and experienced with individual and group presentations, in person and virtually
Knowledge of and sensitivity to the impact of social, economic, environmental, and cultural issues in diabetes self-management
The ability to speak multiple languages would be considered an asset.
Class G License and vehicle available for use. Travel is an expectation as this position will be involved in outreach activities
In this role you will be expected to complete the following tasks:
Provide diabetes education to individuals with diabetes.
Maintain a large and busy caseload.
Meet 6-7 patients daily for initial referrals and follow up appointments
Prepare, plan, deliver, and document weekly classes using hybrid options (a mix of in-person and virtual) for up to 25 people per group.
Provide education related to diabetes and the disease process; medication, diet, nutrition, foot care, healthy lifestyle, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection.
Review blood glucose readings for the purpose of medication control and adjustment
Provide ongoing assessment and recommendations to the treatment team including the patient's healthcare providers and caregivers for adjustments in the plan of care as required
Document assessments, interventions and care plans including recommendations and goals, progress notes and discharge summaries

The successful candidate would be eligible for the following benefits:
Starting Salary of $33.07-$39.25 Hourly
Paid 30-minute lunch for shifts exceeding 5 hours.
5 Sick/Personal Days per year (pro-rated to hours worked)
Enrolment in Healthcare of Ontario Pension Plan (HOOPP)
