We are currently looking to hire a:

Diabetes Dietician Educator



1 - Full Time, Permanent (35 hours per week)



Reporting To:

Manager, Inter-Professional Primary Care

Purpose:


SCHC is seeking a Diabetes Dietitian Educator to join our Diabetes Education Program, composed of Registered Nurses, Registered Dietitians, a Social Worker, and administrative support. The program educates and counsels clients at risk for diabetes, pre-diabetes, and type 2 diabetes in both individual and group settings.

The Diabetes Dietitian Educator will deliver evidence-based, culturally sensitive education to empower clients and families to effectively self-manage diabetes/prediabetes. The role includes program implementation, evaluation, and collaboration within an interdisciplinary Community Health Centre model of care.

Key Responsibilities:



Client Interaction:



Conduct comprehensive assessments of clients' knowledge, readiness to change, socio-economic context, cultural background, and health literacy to develop individualized care plans.

Apply adult learning principles, motivational interviewing, and health coaching to promote behavior change and self-management skills.

Monitor clinical outcomes (e.g., HbA1c, weight, blood pressure) and evaluate the effectiveness of client interventions.

Maintain accurate medical records and communicate care updates to clients' primary care providers and other healthcare professionals.

Program Development and Evaluation:



Collaborate in planning, implementing, and evaluating the community-based diabetes education program to meet the needs of Scarborough's high-risk populations.

Develop and maintain culturally tailored educational materials, lesson plans, and resources.

Participate in quality improvement initiatives, audits, and program evaluation to enhance service effectiveness.

Plan and conduct community outreach, screenings, workshops, and health promotion activities.

Advocacy and Leadership:



Advocate for the needs of diverse and underserved populations, identifying gaps in service and tailoring interventions accordingly.

Mentor, train, and supervise students, interns, and new staff in diabetes education best practices.

Serve as a resource for colleagues, healthcare providers, and community organizations.

Partnerships and Professional Development:



Build and maintain collaborative relationships with community agencies, healthcare providers, and stakeholders to enhance client support and program reach.

Participate in professional networks (e.g., CDENT, DES) and pursue ongoing education to maintain and enhance professional competence.

Promote inter-agency referrals and contribute to SCHC's mission, values, and vision.

Qualifications:



Registered Dietitian, in good standing with the College of Dietitians of Ontario.

Certified Diabetes Educator (CDE) preferred.

Certification in Adult Education principles considered an asset.

Experience:



3-5 years of practice in diabetes education, preferably in community or primary care settings.

Experience with individuals with mental health conditions and diverse populations preferred.

Demonstrated experience in program development, implementation, and evaluation.

Skills and Attributes:



Sound knowledge of evidence-based diabetes prevention, management, and care strategies.

Strong presentation, group facilitation, and communication skills.

Cultural sensitivity, awareness, and ability to adapt programs for diverse populations.

Proficiency with electronic medical records and Microsoft Office applications.

Strong organizational, interpersonal, and problem-solving skills.

Ability to work independently and collaboratively in interdisciplinary teams.

Valid Ontario driver's license and access to a vehicle.

Current CPR/First Aid certification or willingness to obtain.

Second language relevant to priority populations in Scarborough (e.g., Chinese dialects, Tamil, Urdu, Arabic, Farsi) is an asset.

Working Conditions:



Flexibility to work occasional evenings and weekends.

Willingness to travel across multiple program sites.

Remuneration:

$34.03 - $40.99
