Job Description

The Alzheimer Society of Sarnia-Lambton (ASSL) is a community support service organization that provides programs and services to people living with dementia and their care partners in Sarnia-Lambton. We work closely with other service providers to ensure quality care and support for our clients.

We employ administrative and fund development staff, educators, social workers, nurses, therapeutic recreation staff, personal support workers and volunteers who work together as a team to meet the needs of people living with dementia and their care partners.

The Day Program Coordinator is responsible for supervision, activation, and support for all participants in the Day Program. They will assist with the implementation and coordination of recreational, social, physical and mentally stimulating activities that will meet the needs of each client, while ensuring a safe environment is maintained. With support from the Manager of Respite Programs, the Day Program Coordinator will direct and supervise the Day Program Support staff to ensure the program operates according to policies, procedures, and guidelines.

What you will be doing

Maintain a current knowledge of Alzheimer's disease and related dementias Support the planning, coordination and delivery of all activities for the day program, providing support and feedback to support workers in the program Schedule Day Program Support Workers to ensure consistent program coverage Participate in the recruitment, orientation and training of Day Program Support Workers and volunteers Develop and implement scheduled activities, considering client interests, needs and abilities, ensuring varied programming and alternatives for clients; ensure programming includes a component of physical activity/exercise Ensure meal planning and grocery shopping are completed, following dietary restrictions, client preference and budget requirements Promote client participation in all aspects of the day program, providing alternative programming or attending to individual needs as required Facilitate all new referrals, and ensure successful transition in the day program Monitor and record daily attendance in the day program Observe and report any changes and/or information pertaining to the well-being of clients according to the agency's policies and procedures Complete client participation reports, or any documentation in accordance with the policies and procedures Report any serious changes or occurrences to the Program Manager Be available to assist clients with individual needs during their participation in the program (provide reminders, prompts, cuing and support) when needed Provide medication support when needed (medication reminders only) Report any serious injury or occurrences to the Program Manager according to policy and procedure Provide ongoing communication with Program Manager to consult on client care plans Support and participate in the on-going evaluation of programming to meet the individual client needs. Schedule and attend staff meetings with day program support staff and Program Manager as well as other interdisciplinary meetings, as needed Promotes a safe environment for both the client and day program support worker Maintain established community partnerships and facilitate new opportunities for collaboration with community partners and stakeholders. Maintain client confidentiality and respect client individuality Remain flexible in approach to programming, to be able to accommodate individual changes throughout their participation in the program Attend to cleaning and sanitization of the Day Program space as required Perform other duties consistent with the job classification, as required Participation in the respite on-call rotation is a requirement of this position.

What you will bring along:

Education:



Post-secondary diploma in Gerontology, Recreation Therapy, Social Sciences, Nursing or other relevant areas of study (acceptable to the employer). Current registration with a professional college is preferred.

Experience:



Experience working with vulnerable populations in a community or group setting Experience in group facilitation and programming Minimum of 2 years' experience working in an older adult day program setting is preferred.

Other Knowledge, Skills, Abilities or Certifications:

Knowledge of Alzheimer's disease and other dementias Ability to work independently and as a cooperative team member Excellent verbal, written and listening skills Analytical skills to assess client's needs, functional level and ability to perform Activities of Daily Living and Instrumental Activities of Daily Living Excellent organizational and decision-making/problem-solving skills Time management skills to follow an established schedule Food handlers' course is an asset Interpersonal skills and the ability to communicate with clients, colleagues, supervisors, agency personnel and other health care professionals. Mindfulness of cultural sensitivity and diversity Ability to speak French or other languages an asset

Travel Requirements:



A valid driver's license and access to a dependable vehicle is required.

Physical Demands:



Performance of physical tasks related to providing personal care (e.g. assistance with transferring clients may be required) and client mobilization Performance of personal hygiene tasks with clients Performance of general housework, light cleaning, cooking, grocery shopping Performance of tasks to engage clients, such as walking, reading, playing games, making crafts Significant periods of standing, moving and bending

What we offer

Hours of Work: This is a one-year contract

. The Day Program Coordinator generally completes all duties within normal business hours (8:30am-4:30pm, 30-minute unpaid lunch); however, flexibility in hours is required, with occasional evening and weekend work.

Salary

: $50,000-$57,000 range dependant upon experience
