Job Description

Summary: Under direction of the Team Lead, the Cultural Support is responsible for providing guidance and support to clients and families. This position will provide cultural and emotional support through the delivery of traditional, cultural and relevant health and wellness programming. Services are delivered within the seven First Nation communities and off reserve clientele.

Job Duties:1. Provide Cultural and emotional support through the delivery of traditional/cultural and relevant health and wellness programmingo Schedule and coordinate visits for clients and families/caregivers; including on-callo Provide health support services for clients/families through intake/referral (i.e. primary care, mental health and addictions, health and wellness, both individual and groupo Maintain records and file management, data collection and e-chartingo Provide wellness checks and follow up with clients/familieso Assist with providing emotional support to clients/families at events and gatheringso Attend and participate in family cultural support and grief circleso Ensure referrals are directed for appropriate service supportso Engage with interprofessional team members and external partners to promote and improve access to programs and services and be an active part of clients' circle of care


2. Provide community education and engagemento Network and consult with other Community Support Providers, to advocate for collaborative health care delivery for familieso Provide information on enhanced and expanded trauma-informed health and cultural services 3. Perform administrative duties in support of the program's goals and objectiveso Input accurate client information and other relevant data into Electronic Medical Records (EMR)o Prepare and provide monthly reports to support program evaluation and provide recommendations for ongoing quality improvemento Assist with planning and support of traditional wellness events and gatherings

4. Support organizational integration, quality improvement and cultural safety o Ensure all work and interpersonal interactions are conducted with respect and cultural sensitivityo Supporting program development in prevention, healing, and aftercare as well as delivering programs and services to clients, families and the community in collaboration with other health and wellness staff o Support the development and implementation of the organization's quality improvement initiatives, toward improving health outcomes, client experiences, program service delivery and program evaluation

5. Perform other duties as assigned within the scope of the position

Requirements:

Mandatoryo Post-secondary diploma in Indigenous studies, social service, health care delivery, community mental health, or other relevant fields and/or related work experience within an Indigenous organization or community o Previous experience with the delivery of cultural support programmingo Demonstrates practical knowledge of traditional Indigenous and mainstream healing practices relating to emotional, physical, intellectual and spiritual wellness o Self-motivated individual with the ability to utilize available resources in a team oriented shared health care environmento Good interpersonal, listening and communication (written and verbal) skills with the ability to engage people to build trust and rapporto Ability to problem solve and take initiativeo Strong organizational skills and ability to work with deadlineso Must be able to maintain confidentialityo Proficiency in using Microsoft Office, including database applicationso Willing to work flexible hourso Valid class "G" license and own reliable vehicleo Ability to travel including clear driver's abstracto Clear Vulnerable Sector Check

Preferredo Ability to speak, understand or willingness to learn the Anishinaabemowin languageo Indigenous Cultural Safety Trainingo Post Traumatic Stress Disorder (PTSD) Trainingo Knowledge of Electronic Medical Records (EMR) databaseo Current CPR Level C and First Aid Certification

Knowledgeo Understanding and sensitivity related to Anishnaabe language and cultureo Awareness of traditional healing practiceso Knowledge of First Nation communities of Manitoulin Islando Knowledge of the cultural services available on the First Nations within the Manitoulin Island/Espanola catchment area
