St. Joseph's Health Care London is the lead for the Regional Geriatric Program of Southwestern Ontario (RGP SWO). The RGP SWO has 5 key areas of focus: i) clinical excellence, ii) education and capacity building, iii) research and evaluation, iv) capacity planning, and v) advocacy. The Consultant, Quality Improvement / Facilitation will utilize various quality improvement and co-design approaches to support development and implementation of evidence-based strategies in an effort to support health system improvements for older adults and their caregivers.
This position will support and manage process improvements/lean projects, implement co-design approaches and work with hospital staff/physicians, older adults, caregivers and community stakeholders to implement best practice strategies, always ensuring there are measurable, sustainable results. The Consultant, Quality Improvement / Facilitation will guide the implementation and execution of PDSA (plan, do, study, act) cycles, assess and evaluate program outcomes, and identify opportunities for quality improvement across the region. They will work closely with the Manager, Regional Geriatric Program.
A secondary focus of this work will be the engagement of community partner agencies and the successful translation of geriatric best practices and resources into materials that the health system partners can use. It is expected the Consultant, Quality Improvement / Facilitation will work closely with cross-sectoral partner organizations & Ontario Health Teams. The role may also represent the Regional Geriatric Program and St. Joseph's Health Care London at a variety of local and provincial committees.
Essential Qualifications
Master's degree in Health Sciences, Public Health Policy, Social Sciences, Occupational Therapy, Physiotherapy or Business
3 years in the healthcare sector with a focus on Quality Improvement and/or Knowledge Translation
Experience in leading/influencing practice change in either a business or health setting
Proven ability to lead patient and family engagement initiatives using an experienced based co-design approach
Proven ability to lead quality improvement projects, with a high competency in teaching/coaching others and in implementing QI tools
Proficient project management skills
Advanced computer skills (Microsoft office, PowerPoint)
Demonstrated excellence in interpersonal, written (including report writing) and verbal communication skills, including the ability to demonstrate tact and diplomacy with varied groups of stakeholders, including frontline staff, physicians, administrators
Experience in engaging and facilitating work with diverse groups of stakeholders including patients, families, senior leaders, front-line staff and physicians
Highly competent and effective presentation and meeting facilitation skills including the ability to present and discuss complex information in a way that enables and establishes rapport, persuades and influences others and gains understanding
Demonstrated understanding of performance measurement and program evaluation skills
Proven ability to work independently with minimal supervision, take initiative and perform will in stressful environments
Superior analytical, interpretive and communication skills
Proven organizational skills with the ability manage multiple projects and constantly adjust priorities Demonstrated experience in adult education, with a solid understanding of adult education principles Knowledge of a Safety Culture in a Health Care Setting in compliance with Occupational Health and Safety Act (OHSA
Preferred Qualifications
Proficiency in French would be an asset
