REPORTS TO: Executive Director WAGE: $23.00/Hour Time commitment 37.5 hours/week Objectives to Meet: Increase public awareness and knowledge about Mental Illness and Diabetes utilizing, Community Kitchen one time per week, with a focus on proper nutrition as a mechanism for prevention and maintenance of Chronic Disease and Mental Illness. Provide awareness workshops once per month with a focus on Diabetes and Mental Illness topics. Encourage the urban population with Mental Illnesses to access the services available through the following actions. o Provide Psycho-educational group sessions once per week through partnership with Mission Mental Health Unit and or community partners, o Provide individual Mental Health support, assessment and referral for clients accessing the program at a minimum of 10 hours per week. Help people recognize Mental health issues and identify Mental Health services available Creating service linages with Mental Health services throughout the Fraser Valley Support the prevention and or delaying the onset of Diabetes in at risk individuals through the following, o Provide one comprehensive workshop on diabetes focussing on awareness, prevention, onset, and management. SKILLS - Good communication skills - Caring, considerate and respectful of clients' needs - Ability to multi-task, meet deadlines and work well under pressure - Ability to keep accurate statistics/records in both written and electronic forms - Tracking attendance and workshops (numbers and effectiveness) - Surveying program clients at periodic intervals (pre and Post) for effect Create health workshops based on up-to-date information and practices for health issues such as, diabetic cooking, mental illness, spiritual guidance, active living workshops, chronic disease, etc. - Maintain client files and record progress of health-related issues - Organize monthly cultural activities and ensure to incorporate Culture into all activities. - Coordinate and implement age-appropriate activities for clients attending the Centre - Develop and maintain working relationship with other community services - Promote and participate in cultural events and workshops - Maintain accurate records of all client participation - Submit monthly/quarterly reports to the Executive Director - Collect accurate statistics on client participation - Attend conferences and events as directed - Coordinate and assist with centre events as required - Must be reliable, punctual, and friendly - Must work well independently or as part of the team KNOWLEDGE - Working knowledge of Microsoft Office, Word, Excel and Outlook - Certificate/Diploma in Social Services 2-3 years of equivalent working experience - Knowledge of Residential School Systems and the impact - Demonstrated ability to work with people from a variety of socio-economic backgrounds - Experience working with people of all ages ADDITIONAL REQUIREMENTS - Criminal Records Check - First Aid (Can provide training) - Food Safe (Can provide training) People of Indigenous descent are encouraged to self Identify
