NATURE OF THE WORK



Reporting to the Community Support Program Coordinator, the Community Support Worker will be a member of the Community Support Team, ensuring the provision of direct services in both official languages to eligible client, performing administrative functions, and liaising with community partners as identified in work plan or assigned by the Community Support Program Coordinator.

RESPONSIBILITIES



Conduct program-related client intakes and needs assessments through individual interviews. Meet with client in person or by telephone, including family/caregiver when required. Liaise with clients on an ongoing basis. Provide information and advocacy to ensure clients receive necessary community health, legal and financial support services. Mobilize and coordinate resources to respond to client needs. Make and receive referrals to ensure service integration. Receive and address client complaints, cancellations or problems tactfully and appropriately. Respond to emergencies identified by workers, clients, volunteers and caregivers. Complete and submit appropriate forms for all clients. Maintain individual client files and ensure confidentiality. Prepare and maintain necessary documentation and statistics. Inform and update the Community Support Services Coordinator on program activities, issues and trends. Recruit, assess, match, and maintain lists for volunteers Coordinate support to clients through volunteers including supervision and quality service delivery. Participate in the design and implementation of service delivery programs. Participate in staff meetings, staff training, team meetings, centre-wide activities, etc. Promote awareness of issues affecting seniors and people with disabilities and serve as a community educator to enhance public knowledge of available services. Establish and maintain formal and informal links with community groups and partner agencies to facilitate referrals and positive working relationships. Respond to community requests for information. Maintain visibility of programs through media, local newspapers, pamphlets, local church groups etc. Coordinate invoicing, collections, deposits, and cheque requisitions associated with programming when required. Provide supervision to students as required. Uphold the philosophy of the Centre.

WHAT WE WOULD LIKE TO FIND IN YOU



B.S.W. University Degree or College Diploma with relevant experience. Two years' experience with the delivery of community support services or programming related to seniors or adults with a physical disability. Demonstrated understanding of the unique needs of seniors and adults with physical disabilities. Demonstrated ability to work effectively in a multi-service team. Ability to work independently and take initiative. A willingness to maintain high quality, client centered services through superior organizational and problem-solving skills. Fluency in oral and written communication skills in English and French.

French language skill is mandatory.

Must submit and maintain a favorable criminal reference check. Must have a valid drivers' license and have access to a motor vehicle. Possess a non-judgmental & egalitarian attitude regarding gender, race, culture, sexual orientation, and socio-economic status.

Pay: $28.49-$35.02 per hour
