Program Description:



CityServeTO is a program designed for youth aged 16-24 who are ready to take the next step but face real barriers like poverty, violence, racism, or involvement with the justice system. This isn't just about earning a paycheque, it's about finding purpose, building community, and being seen. Through hands-on service, personal growth, and employment, participants gain the confidence, skills, and support needed to shape a new path.

CityServeTO isn't just a job program - it's a purpose-driven journey that offers youth a team to lean on, a place to belong, and the tools to build a better future.

In partnership with a grassroot organization, The Forgiveness Project, this project offers a unique opportunity to work collaboratively with their extensive experience in supporting individuals affected by community violence.

Purpose:


The Community Support Coordinator helps young people develop the confidence, skills, and relationships that can shift their path and their potential.

Key areas of responsibility:


The primary responsibilities of the Community Support Coordinator position include, but are not limited to:

Lead the coordination and delivery of the program.

Provide case management support to participants, ensuring each young person has a personalized growth plan.

Facilitate workshops and team-building activities that develop life and employment skills.

Coordinate logistics, partnerships, and schedules to keep programming running smoothly.

Evaluate program outcomes and identify opportunities for innovation and impact.

Support organizational performance improvement through data-driven insights and collaborative planning.

Oversee the delivery of all aspects of the program including the execution of the City ServeTO program.

Develop and update program materials as needed and coordinate the development and distribution of necessary documentation.

Collaborate with City of Toronto Community Development Officer and other internal and external partners to build relationships etc.

Provide mentorship, case management, and wrap-around supports to the cohort participants to ensure successful completion of the program.

Work with the Senior Leadership team to collect, track and report on key program deliverables and ensure that all reports, documents, work plans are completed on time.

Raise awareness of the program using presentations and connect with community agencies to develop and maintain partnerships and create referral pathways, if needed, after completion of the 40-week program.

Ensure culturally relevant and holistic approaches are embedded in program delivery.

Work with evaluation team to develop, monitor and evaluate the program and share key learnings for future cohorts.

Educational and Professional Qualifications:



Possess a diploma or degree in Social Services, Child and Youth Work, Social Work, or a related field from a recognized post-secondary institution is preferred, or an equivalent combination of education and experience.

At least 1 year of experience coordinating and delivering youth programs.

At least 1 year of experience working with racialized youth.

Experience in program development, implementation, and evaluation.

Knowledge of how trauma, stigma, culturally insensitive care, systemic oppression, and anti-Black racism impact and create barriers for Black youth and other racialized groups, particularly those involved with the justice system.

Community base-service knowledge for priority populations, caseload management experience, including developing action plans for clients, monitoring client progress and maintaining records.

Proficient communication, facilitation, and networking skills.

Lived experience, along with a strong understanding of and demonstrated experience applying anti-oppressive and anti-racist frameworks in practice.

Proficient with computer software including MS Office, Internet, email such as M365, Outlook, SharePoint, MS Project, MS Visio, OneDrive, and SharePoint, etc.

Demonstrated skill in conflict resolution by addressing issues proactively, facilitating constructive discussions, and finding mutually agreeable solutions to maintain positive working relationships and project momentum.

In-depth knowledge of Greater Toronto Area (GTA) neighbourhoods and communities, with an understanding of their cultural diversity, unique social dynamics, and the availability and accessibility of local services and resources.

Remuneration:

$25.60 - $28.91
