Job Summary




The Queen's Family Health Team is a multidisciplinary team of health professionals who provide primary health care services to the Kingston community, who train future primary health care professionals and who conduct research in issues of primary health care delivery. The Queen's Family Health Team is comprised of a number of medical practices and provides for a diverse, client centred work environment for all employees.



Reporting to the Nursing Manager of the Queen's Family Health Team, the Community Services Worker provides community based social services to identified groups, individuals and families within applicable legislation and in accordance with the policies and procedures of Queen's Family Health Team.



KEY RESPONSIBILITIES:



Provides individuals and families with social services and support intended to improve their social well-being and functioning, for example, provides community resource information to patients, makes connections and appointments for them as needed and may also meet them in the community to support them as they access other community resources. Assists patients and families in accessing resources and assistance including food, housing, forms completion, ODB and other health resources/benefits. Provides information and referrals regarding community services in the area (e.g. food bank, housing appointments). May be required to conduct home visits, and will conduct home visits with Health Link Registered Nurse (RN) to support any referrals the RN feels may be required. Liaises with external resources in support of patients to increase probability of obtaining community resources. Listens to the patients, assesses their needs and determines based on her assessment how best to support them and where to redirect them. Works collaboratively within a multi-disciplinary team environment as well as with community professionals. Coordinates seasonal programs such as Christmas hampers and other community and family service programs. Completes accurate and timely documentation in patients' echarts on the encounter with the patient Undertakes other duties as required in support of the Department.


REQUIRED QUALIFICATIONS:



Completion of social sciences undergraduate degree from a recognized University,

or a two-year diploma in social sciences from a community college.

Previous (five years preferred) related experience in the social services. Satisfactory Criminal Records Check and Vulnerable Sector Screening required. Understanding of medical terminology. Current CPR would be considered an asset. Access to a vehicle with valid G class driver's licence and insurance, would be

considered an asset.

Consideration will be given to an equivalent combination of education and

experience.



SPECIAL SKILLS:



Strong organizational and self-management skills. Strong interpersonal and communication skills (both verbal and oral) and ability to

work within a team environment.

Ability to problem solve and de-escalate potentially crisis situations. Ability to work with a variety of populations including persons with intellectual

disabilities, mental health disorders and groups with varying levels of

comprehension and language capacity.

Ability to work in a variety of settings such as community support agencies, and in

the patient's home.

Sensitivity to cultural, religious, language and gender differences. Demonstrated interviewing and assessment skills. Knowledge of local community support services, mental health services and ability

to access these resources for patients.

Proficiency in computer software programs including electronic medical records,

word, excel, e-mail, and internet search methods.

Ability to prioritize, manage time effectively and be flexible in a fast-paced setting. Ability to work effectively with minimal supervision and as a team player. Maintains privacy, confidentiality and security of patient information.


DECISION MAKING:



Prioritize work and time. Respond to general inquiries, and provide reasonable answers, or a means to finding

answers if necessary. Redirect to other individuals/clinicians as required.

Determine best way to provide information about resources to patients and the

community.

Patient-centred problem solving which may require situational risk assessment.
