EFry Hope and Help provides trauma-informed, inclusive and gender-responsive programs and services to support women and girls at risk or involved in the legal system in building stable lives
We are recruiting a Community Re-entry Caseworker. The Women's Mobile Support (WMS) Caseworker will provide a continuum of care for women exiting out of the Vanier Centre for Women and support additional identified women from the pre-discharge and discharge to stabilization with flexible operating hours. The WMS Caseworker will provide a strong mentoring relationship and encourage safe and healthy decision-making, nurtures independence and skill development, and provide connections and referrals to community resources required for successful reintegration. The WMS Caseworker will work specifically (collaboratively) with the Community Organizations and Probation Services in providing a coordinated approach that will ensure a greater impact. In providing these supports, WMS aspires to establish community linkages to address social and justice related issues such as mental health, addiction services, housing support, education, employment, and basic needs.
The WMS Caseworker must be available to work out of various locations including but not limited to Brampton, Mississauga, Halton Region and across York Region. There is potential that the WMS will be required to travel to Hamilton, London and Lindsey as part of reintegration planning. The hours for this position may vary from Mondays to Fridays and evening hours may be required.
KEY RESPONSIBILITIES:
Assess women using the SPDAT (Service Prioritization Decision Assistance Tool) and EFry's resources for applications related to Identification, Income, other entitlements; and arrange housing or alternate shelter discharge will be coordinated.
Provide individually tailored services to each client, e.g., housing placement, mental health, addictions, vocational guidance, appointment escorts, assist with basic needs
Assist with tools for managing money; prepare budgets and compute expenses with individuals in the program.
Coordinate and monitor referrals to community services, and advocate for individual's participation in them.
Knowledge of women's issues including criminogenic factors.
Provide linkages to EFry gender responsive programming and workshops.
Conduct follow-up for clients that have exited the institution as part of aftercare
Demonstrate the ability to work within a fast past environment with ongoing interruptions and disturbances. Comply with institutions protocols
Maintain written and computerized records, compile reports and complete other program documentation (including case notes, statistics, letters, and among others.)
Document housing placements, follow-up and aftercare to maintain housing, and rehousing.
As needed meet women in community to support with aftercare and follow up
Develop transportation plans, or transport, perform other related duties as directed.
Performs other related duties as required.
QUALIFICATIONS:
A degree and/or diploma in a related field with experience working with vulnerable populations (or equivalent life experience)
Knowledge of the Vanier Centre for Women and demonstrated awareness of relevant legislation including but not limited to the Canadian Criminal Code, and the Child and Family Services Act
Knowledge of community resources and services
Practical knowledge and experience administering gender specific and responsive programming
Excellent planning and time/case management skills
Ability to work independently, and as a team player
Excellent documentation and computer skills, as well as organizational and time management skills.
Strong communication (verbal and written) and interpersonal skills with the ability to work well with
diverse community groups and other stakeholders.
Proficiency in French language is considered an asset. Interviews clients, prepares case histories, assesses needs and outlines services provided by the organization. Refers clients to appropriate programs.
MANDATORY JOB REQUIREMENTS:
Standard First Aid / CPR certification.
Evidence of COVID-19 vaccination status.
Satisfactory Criminal Record Review (Vulnerable Sector Search).
Ability to lift 30 pounds.
Doctors note of fitness to perform the duties of the job.
Valid Ontario driver's license, with satisfactory driving record. Must possess a valid Ontario driver's license in good standing and have use of a vehicle with appropriate insurance
