Wage:
$22.66 to 26.62/hour; based on experience working within a similar population/setting.
Reporting to the designated Program Manager, the Case Facilitator is responsible for all direct support, coordination, planning, development, implementation of programs and their evaluation, for designated RBs in accordance with the Agency's mission statement, philosophy, givens, policies, procedures, and all relevant legislation.
MAIN ACCOUNTABILITIES
Develop, implement and revise Individual Support Plans for agency participants. Identify needs and goals and create a support schedule that will provide opportunities to assess outcome measures.
Support, direct and advise participants according to the agency philosophy by adhering to the shift expectations, assisting participants with all activities of daily living, transfers, household management activities, and supervising medication administration.
Completing Functional Assessments and ensuring that all necessary arrangements for required resources, staffing, and transportation are completed.
Accompany participants to appointments, meetings, or events as requested or directed; provide portering assistance and supervision to activities.
Communicate effectively with Rehabilitation Counsellors by sharing current information about participants through counsellor notes, communication binders, observation sheets, staff messages, voice mail, e-mail and attending meetings with all interested parties.
Must be physically and mentally capable of performing the duties of the position and have an excellent rapport with participants, co-workers and community partners
REQUIREMENTS
Degree or diploma in social services, psychology, sociology, social work, or related field.
Minimum of two years' full-time equivalency experience in social services developing individual support plans or two years' full-time equivalency experience as a Rehabilitation Counsellor at BICR (or a combination thereof totaling two years' full-time equivalency)
Demonstrated proficiency in Microsoft Office based programs
High level of demonstrated problem solving and organizational skills
Knowledge of community based supports and services in the Niagara Region
Valid driver's license and access to reliable vehicle.
Current CPR/First Aid certification.
Proficiency in speaking/reading French a preferred asset.
PHYSICAL REQUIREMENTS & WORKING CONDITIONS
Must be physically and mentally capable of performing the duties of the position.
Working as a team and independently within various settings, including residential and the community.
May be exposed to infectious bodily fluids, diseases, odours, conditions, etc...; exposure to the threat of physical or verbal abuse; direct and personal responsibility for the care, welfare and safety of the individuals we support; direct intervention in crisis and emergency situations.
Intermittent physical activity including walking, standing, sitting, lifting up to 50lbs, pushing/pulling wheelchairs, transferring participants, assisting with swimming and physiotherapy routines; manual dexterity required to use computers, kitchen equipment, etc....
Regular travel throughout Niagara Region.
Must be flexible to work various shifts as needed, including day, afternoon, evening, overnight awake, overnight asleep and holiday shifts on rotating basis.
