Reporting to the Health and Wellness Manager, the Community Outreach Supervisor plays a pivotal role in overseeing SEEDReach, a Mobile Addiction and Mental Health Outreach Team. In this role, you will provide leadership, guidance, and support to a multidisciplinary team, ensuring they operate from a trauma-informed, client-centered, and holistic perspective. Your mission will be to address the needs of individuals experiencing chronic and episodic homelessness, mental illness, addiction, and other stability challenges, ensuring their basic needs are met through street-based interventions and integrated case management. You will also manage a small caseload of clients with complex concurrent disorders, offering support to social workers in treatment planning, navigation, and resource provision. Collaboration with internal and external stakeholders is crucial to ensuring effective care and service delivery.

Position Type: Full time, Fixed term until April 30, 2024
Schedule: Monday to Friday, 8:00 am to 4:00 pm
Location: Wellness Centre, Calgary, AB
Reporting to: Health and Wellness Manager

  • Provide leadership, supervision, training, and support to the outreach team.
  • Collaborate with the multidisciplinary team to offer comprehensive case management to clients.
  • Provide assessment and counseling support to clients with mental health and substance dependency needs.
  • Assist clients in optimizing their utilization of available community supports.
  • Facilitate weekly case consultation meetings for SEEDReach staff.
  • Efficiently manage team schedules and client appointments.
  • Contribute to the development and enhancement of outreach programs.
  • Support the development and facilitation of group sessions and workshops addressing mental health and addiction.
  • Foster partnerships and collaborations with community organizations and government agencies.
  • Identify grant opportunities, prepare grant applications, and secure funding.
  • Manage two distinct funding streams to ensure program financial stability.
  • Review funding requests for eligibility and compliance with contracts.
  • Prepare and submit regular reporting to internal and external stakeholders.
  • Ensure the program maintains up-to-date data collection practices.
  • Facilitate emergency interventions as needed.
  • Document client interactions, referrals, and progress in relevant systems.
  • Assist in the planning and execution of ID Clinics.
  • Engage in leadership, committee, and community meetings.
  • Contribute to the development and evaluation of Health and Wellness policies.
  • Stay updated on best practices in social work, homeless outreach, and related fields.
  • Provide collaboration, consultation, and support to internal teams.
  • Offer input into program evaluation upon request.
Minimum Qualifications and Skills
  • A passion for ending homelessness in Alberta.
  • Bachelor\'s of Social Work (BSW), Master\'s Degree in Social Work (MSW), or equivalent with active registration in good standing.
  • Minimum four years of experience working with vulnerable populations and homelessness.
  • Proven record of mental health assessment, counseling, and ability to implement a plan of care.
  • Effective communication skills, both verbal and written.
  • Demonstrated knowledge of Calgary\'s community mental health and addiction resources.
  • Knowledge of FOIP and ability to observe confidentiality standards.
  • Self-starter with the ability to work in a cross-functional team.
  • Proficient computer skills (Microsoft Office Programs); experience with Homeless Management Information System (HMIS) is an asset.
  • Strong organizational, time management, and advocacy skills.
  • Excellent written, oral, and interpersonal skills.
  • Valid Driver\'s License (Class 5) and access to a reliable vehicle.
Working Conditions
  • This position involves a mix of office working conditions and street outreach.
  • May entail working in extreme weather conditions; daily weather awareness is essential.
  • Physical requirements include lifting, traveling, sitting, standing, walking, and driving a personal vehicle.
  • Moderate physical and mental effort required to accomplish essential job functions.
Additional Requirements
  • This position requires the use of a company vehicle. Candidate must have and maintain valid driver\xe2\x80\x99s licence (class 5) and will need to consent to a driver\xe2\x80\x99s abstract request.
