We are seeking a dedicated and compassionate Community Nurse to join our healthcare team. Duties

Deliver comprehensive nursing care to patients in various community settings, including homes, clinics, and health fairs. Assess patient health status through physical examinations and medical history evaluations. Develop and implement individualized care plans based on patient needs and preferences. Educate patients and their families about health management, disease prevention, and treatment options. Collaborate with multidisciplinary teams to coordinate care and improve patient outcomes. Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with healthcare regulations. Stay updated on best practices in nursing and community health initiatives.

Requirements

Valid nursing license (RN or LPN) in the state of practice. Strong knowledge of patient care principles and nursing procedures. Familiarity with medical terminology is essential for effective communication with healthcare professionals. Excellent interpersonal skills to build rapport with patients and families. Ability to work independently as well as part of a collaborative team. Previous experience in community health or home health nursing is preferred but not mandatory.
