Job description The goal of the Churchill Harm Reduction Project is to work together with local agencies to provide trauma-informed, social prescription, asset-based support , including: health, food, financial literacy, housing, life skills, clothes, household needs, mental health, re-integration and harm reduction supports to reduce the number of overdoses, emergency calls and increase quality of life. We support people living in poverty, people who are engaged in sex trade work, people struggling with addiction, and homelessness. Our agency roll is to coordinate local agency supports, run food programs and provide harm reduction and navigation type supports. Qualifications: College Diploma or University Degree in Social Work/Community Work/Social Justice/harm reduction. Suitable clients will have completed at least two years experience in the field Vulnerable Screen Driver licence and access to a vehicle. Duties: Client case management support that is client-led, using Social Prescription, Determinants of Health and ensures dignity Develop overdose plans with clients, both one-on-one & group Share best practice strategies with all partners on harm reduction Monthly program schedule of all agencies Provide agency use of space co-ordination Community outreach Organize, and support partner meetings Run Food Support programs with volunteers Support Client goals and track outcomes Plan client activities/programs Increase the number of clients with a doctor Reduce the number of emergency calls due to over doses through harm reduction distribution Track outcomes and report harm reduction distribution statistics, complete funder reports.
