Remote work or in office if preferred - Office located in Collingwood

Working as a Community Navigator will provide you an opportunity to address real and systemic barriers to services that may be amplified by poverty, inequity and intersecting personal identities. This position is fast-paced and outcomes oriented. It requires an ability to adapt and learn and an appreciation for the root causes of inequity and lack of access to services.

As a Community Navigator, you will:



Community Navigators serve clients by phone, text, chat and email by connecting clients with essential services and resources, advocate for their needs, and ensure they receive support. Navigators use trauma-informed and culturally competent practices to assist individuals who may be experiencing homelessness, mental health challenges, poverty, addictions, aging, racial and ethnic discrimination, social isolation and life transitions.

What success looks like:



You connect diverse groups of people by phone, chat, text, or email, to health and human services using a comprehensive database of resources and agency tools; You conduct thorough assessments of client needs, ensuring they are directed to the most appropriate service or program, assist in problem solving and track calls to illustrate community trends; You apply active listening, motivational interviewing skills and anti-oppressive communication to identify appropriate resources and provide information on eligibility requirements and service availability, recognize, and respond appropriately to individuals who are in crisis, and identify those individuals who are vulnerable; At times, you may need to advocate on behalf of individuals who face personal and/or systemic barriers; You will communicate with empathy, respect, sincerity, care, and concern. You accurately input data into the client management system while handling real-time conversations

Requirements:



Ability to work overnight, evenings, days, weekends, and statutory holidays with consistent hours; Post -secondary diploma in or degree in mental health, social work, health or other related fields preferred; Experience in case management and trauma-informed practices and their application is considered an asset; Ability to work with multi-technology, multi-screen and multi-software programs; Valid driver's license and access to a reliable vehicle; Acceptable Vulnerable Sector Check
