Contribution:

The Community Inclusion and Outreach Support worker assists adults living with a developmental disability in developing skills needed to function more independently in the community.

Qualifications:

Minimum Grade 12 with additional coursework in services to persons with a developmental disability. Experience supporting individuals with developmental disabilities in a broad range of community activities. Preferably a combination of relevant experience and education. Valid First Aid Certificate. Valid Driver's License.

Job skills and abilities:

Good communication skills, written, verbal and non-verbal. Ability to work independently, with flexibility and an eclectic approach in order to meet the varied needs of the clients. A commitment to and interest in the field of service to people with developmental disabilities.

Responsibilities:

A. Program

Relate to clients in a professional, caring and positive manner, which represents their personal worth, dignity and rights. Assist client to identify dreams and develop an Individual Service Plan. Provide opportunities for individuals to learn life skills (i.e. personal hygiene, safety, financial management, nutrition and cooking, and home making) with an emphasis on increasing independence. Familiarize clients with community resources including transportation services, banks, shopping and appropriate service resources. Assist clients in developing interpersonal and social skills. Act as an appropriate role model. Work with individual community members, resources and agencies to foster opportunities for inclusion of individuals with diverse needs.
B. Organization

Keep accurate records and submit quarterly reports on client's progress as outlined in the Program Operating Manual. Attend staff meetings, meet with Manager of Community Living and update on program issues, as required. Meet with Ministry representative as required. Assist Community Living Services Manager with maintenance and revision of Program Operating Manual. Other related duties as required.

Requirements:

Criminal Record Review. Signed confidentiality form. Approved reference check. Access to a reliable vehicle. Driver's abstract.

Vacation & Leave:

Employees accumulate 6% vacation pay on all hours worked in the first and second year of employment. After the first two years of employment, they earn one additional day of vacation per year of service, to a maximum of 35 workdays.

When eligible, an employee will be enrolled in the Municipal Pension Plan with a matching contribution by SSCS.
