The Community Inclusion Worker is responsible for the overall well-being of the client, while developing social, employment, and community skills. Community Inclusion Workers act with a client-first mentality to ensure the client's needs are met at all times and the safety of the client is of first priority.
QUALIFICATIONS
Diploma in Social Work, or a related field, and/or minimum two years' related experience, plus personal suitability
Certifications: First Aid and CPR certificate, MANDT Certification (we can train those who require this).
Must demonstrate clear, direct, and professional verbal and written English communication skills
Must demonstrate experience with client goal setting and attainment, personal scheduling, and time management
Must demonstrate ability to model appropriate behavior to clients, such as professionalism, ethical judgment personal integrity, self-awareness and positive social interaction, in order to obtain volunteer work
Must demonstrate a strong sense of personal boundaries and ability to enforce those boundaries respectfully with client and caregivers
Must demonstrate the ability to relate supportively to a person with developmental disabilities and/or mental health issues
Must demonstrate knowledge of different augmented communication approaches such as PECS, ASL, or communication technology or as appropriate for the client
Must be able to work effectively and collaboratively with community resources and within a team setting
Must demonstrate resourcefulness to seek out appropriate support for client, caregivers, and/or self
Must be able to pass a Criminal Record Check
DUTIES AND RESPONSIBILITIES
Planning & Administration
Completing and submitting Incident Reports accurately and objectively within given deadline to Supervisor
Developing and adhering to a monthly schedule with client, caregiver, team, and community including activities to support community inclusion and client goal attainment
Maintaining all records and documentation requested by the Supervisor with accuracy and professionalism
Providing sufficient time to those required for cancelled shifts
Evaluation & Decision Making
Making decisions that align with client goals, needs, and organization policies and procedures, and displaying ability to work autonomously
Maintaining a calm, patient, and respectful demeanor during times of client escalation or challenging situations
Rescheduling activities to best suit in the moment client needs
Researching and developing resources/supports/activities to best meet client needs by actively taking initiative
Client Specific
Administering medication and/or client supports according to client support plans
Encouraging, inspiring, and mentoring social relational skills, employment skills, and appropriate behaviors while engaging with community as a role model to the client
Completing any documentation relating to the clients progress as requested by the client, caregiver, team, and/or Supervisor
Cultivating and sustaining a mutually trusting relationship with client, caregiver, team, and community
Supporting client to cultivate his/her own relationships, while cultivating and sustaining a mutually trusting relationship with client, caregiver, team, and community
Working together with home share caregiver and client to creatively design options base on client preferences, affinities, and suitability
Communication
Advocating for the client to the appropriate party and actively seeking support to achieve client needs
Engaging in continuous communication with the client's team, which may be inclusive of doctors, paraprofessionals, home share, supervisor, and other Community Inclusion Workers
Maintaining accurate records of client progress, setbacks, behaviours, and needs, and communicating these records in a timely manner to the Supervisor or other necessary parties
WORKING CONDITIONS
40 hours/week, Monday-Friday 830-430 at our Surrey Day Program
Salary:
$26.00/hr plus benefits
