Summary of Duties:



Under the supervision of the Nurse Manager, the Community Health Nurse (CHN) works within an interdisciplinary team to provide nursing services to community members and families. As part of the Community Health team, they will mainly work to provide immunizations to school-aged children, adults and elders and participate in other communicable disease prevention work. The CHN also supports maternal and child health programs, including prenatal and postnatal care, newborn visits, and early childhood health monitoring in collaboration with the Maternal Child Health team.

Key Deliverables and Expectations:



Perform all duties and responsibilities in accordance with British Columbia College of Nurses and Midwives (BCCNM) standards and guidelines and Liwat Nation formal policies, standards, procedures, and standard practice. In consultation with Nurse Manager performs duties as directed and implements and evaluates programs. Establish a therapeutic relationship with the client or family using interpersonal and interviewing techniques, in person and/or virtually, to ensure their choice and autonomy in decision-making and care planning. Use their professional knowledge to work in partnership with the client and family to establish wellness goals that are safe, realistic, and reasonable. Promotes client and family empowerment and independence using a strengths-based approach to health education, and health monitoring. Delivers communicable disease control (CDC) program including communicable disease surveillance, immunization programs, outbreak management, TB screening, and community education about communicable diseases and prevention. Includes administrative tasks like reporting, contact tracing, and referral. Provides education, support, and referral to individuals with chronic disease. Health Education for general community members based on assessed needs and health trends. Through health centre visits, clinics, or home visits, deliver direct nursing care per BCCNP standards and Lil'wat Health and Healing guidelines and provide care management to assist clients to manage their own care and navigate through various health services. Promote programs and educate the community about internal and external programs and services and work to ensure access for eligible community members. Creates supports and delivers culturally relevant activities to educate and inform community members on communicable diseases, vaccines, health promotion, and injury and illness prevention. In consultation with Maternal Child Health Coordinator and the team performs duties and supports programming including prenatal and postnatal health, newborn home visiting and assessment, breastfeeding support and ongoing monitoring and support for children birth to 6 years and their families. Maintain collaborative partnership with pharmacist to provide support with Indigenous health, community-based projects and engagement, and initiatives related to cultural safety and decolonization of pharmacy practice. Ensure client documents are maintained and updated to include all required information and ongoing client charting. Completes all required reports and maintains a database of community health information. Perform other related duties as requested.

Experience and Attributes:



Current practicing registration, in good standing, with the British Columbia College of Nurses and Midwives (BCCNM). Degree in Nursing from a recognized university. Current BCCDC Immunization certification. Infant and child immunization experience is an asset. Current CPR/AED for healthcare providers. Demonstrated knowledge or Experience working in Public Health or Community Health, First Nations health issues and health systems. Maintains provincial professional registration and professional liability protection. Experience or working knowledge of perinatal health, child and youth health, communicable diseases, and immunizations. Experience working in a rural setting will be a benefit. Case management experience and experiences as a liaison with other health services will be advantageous. Must possess and maintain a valid Class 5 BC Driver's License, clean driver's abstract and reliable vehicle. (Mileage reimbursement will be paid when personal vehicle used for local business.) * Must be able to provide a clear Criminal Records Check, Vulnerable Sector.
