Ontario Health Teams (OHT) are a new way of organizing and delivering care that is more connected to patients, clients, families, and caregivers in their local communities. Under OHTs, health care providers (including hospitals, doctors and home and community care providers) and social service providers work as one coordinated team - no matter where they provide care. At maturity, each OHT will be responsible for delivering care for their patients/clients, directly connecting them to the different types of care they need and providing 24/7 help in navigating the health care system. Archipel Ontario Health Team Archipel OHT was officially created by 25 multi-sectoral partners at the end of 2019 under the name Ottawa East Ontario Health Team (OE OHT). In late 2022 the members of the OE OHT and the Prescott Russell in development Team announced their alliance to form the Archipel Ontario Health Team (OHT). The Archipel OHT is now comprised of more than 70 partners including healthcare organizations, community support services, hospitals, long term care, palliative care, home and community care, and primary care. Archipel OHT serves diverse communities across unceded Algonquin Territory, from downtown Ottawa to Hawkesbury East, and from the Ottawa River to the southern border of the Prescott-Russell United Counties, providing bilingual services in urban, semi-rural and rural settings. Over 200,000 people live in the area served by our OHT. With its many partners, Archipel\xe2\x80\x99s mandate is to better respond to citizens\xe2\x80\x99 needs by optimizing the planning, coordination and delivery of services in both official languages. Job Summary: Community Engagement Lead As part of the new, dynamic OHT model, the Community Engagement Lead will be responsible for the recruitment/retention, training, education, on-boarding and management of patient/client and caregiver partners. This position will work closely with the co-leads of the Patient, Client, and Family Advisory Committee (PCFAC) to identify priorities, develop initiatives, and drive community engagement initiatives across the OHT. The position also supports and facilitates engagement and collaboration with community partners, and leaders from across sectors by providing training and education regarding people-centred care and patient engagement. The Community Engagement Lead will be responsible for developing engagement strategies for diverse stakeholders and organizations internal and external to the Archipel OHT, in coordination with the Population Health Manager. The Community Engagement Lead is strategically oriented to build meaningful engagement with patients, caregivers, community members and community organizations from across the Archipel OHT, with a specific focus on equity. Summary of responsibilities Reporting to the Population Health Manager, the Community Engagement Lead will be responsible for the following deliverables by March 31, 2024.
Patient/client and stakeholder relationship management and supports
Coordinate and support the Archipel OHT Patient Client Family Advisory Committee (PCFAC), including scheduling and facilitating meetings, drafting agendas, creating slide decks, coordinating presenters, setting up meetings, administering compensation and recognition, drafting correspondence, and implementing evaluation in collaboration with the PCFAC co-leads.
Act as the key point of contact for client partners and OHT partners to identify and resolve questions, complaints or concerns with the engagement processes of the Archipel OHT.
Ensure psychological safety of the patient/client and caregiver partners of Archipel OHT and proactively identify necessary supports to improve the involvement of our lived experienced partners while volunteering with the OHT.
Build capacity among client partners and OHT partners by developing tools and resources including manuals, handbooks, and policies that the Archipel OHT can use for engagement, outreach, and education.
Develop, build and sustain meaningful relationships with diverse stakeholders representing the Archipel OHT communities.
Strategy
Undertake literature reviews, environmental scans on Best Practices in patient/client engagement.
Participate on the Archipel OHT PCFAC and support and lead project-based work, as appropriate.
Lead the design of a local and inter-regional community engagement strategy and ensure strategic alignment with provincial direction.
Engage with stakeholders to ensure diverse perspective and voice are included in all efforts.
Assists in the development of engagement goals and objectives, workplans, policies, and procedures that support the vision, mission, and commitments of the Archipel OHT.
Leverage the Ontario Health\xe2\x80\x99s Equity, Inclusion, Diversity and Anti-Racism Framework to guide engagement work, develop engagement approaches to create mutually beneficial partnerships, and ensuring representation from underserved groups to advance health equity objectives.
Project Administration, Planning & Management
Provide project management leadership and support for the OHT\xe2\x80\x99s PCFAC and its co-leads
Develop, lead, and monitor community engagement work and project plans
Liaise with internal and external stakeholders to smoothly operationalize the digital health strategies of the OHT
Prepare and deliver reports, spreadsheets, letters, presentations, visualizations, and documents for OHT stakeholders as required
Respond/coordinate response to stakeholder comments and concerns
Is a member of the project team that is advancing Ontario Health East region\xe2\x80\x99s Patient Portal strategy
Assist in coordinating patient portal discovery activities and will work in collaboration with internal stakeholders, liaison with Ontario Health Teams (OHT) and Primary Care partners during project implementation.
Educations and Qualifications
Post-secondary degree/diploma in related field (e.g., health, education, volunteer management, social services)
Extensive training and/or professional experience in patient /client engagement, client and person-centred care, education, volunteer services, and/or client relations.
Experience working with senior level executives and/or a diverse range of health service providers
Project management experience considered an asset
Demonstrated ability to build rapport and maintain relationships with diverse stakeholders (i.e., clients, family members and partners)
Knowledge & Experience
Extensive experience engaging with diverse clients, families and additional stakeholders across sectors to build relationships and improve collaborations.
General understanding of foundational OHT concepts including health equity, patient-centered care, collaboration, and collaborative decision making
Good understanding of the Ontario healthcare system and the purpose and function of Ontario Health Teams
Demonstrated understanding of the health sector, including primary care, acute care, home care, long-term care, and community support services
Understanding of change management principles
Abilities

While currently a remote working position, this position will require some travel within the Archipel OHT area for office-based work and/or meetings
