POSITION SUMMARY
As a Clinical Pharmacist, you will be providing the following services to your patients and inter-disciplinary team:
Direct patient care: Communicate with patients and the health care team to determine goals of therapy and collect information; identify drug related issues; formulate and implement a pharmacy care plan and monitor outcomes; provide medication information and care transition
Use your pharmacotherapy knowledge to respond to drug information requests and assist in the development of drug therapy guidelines
Work with FNPCI Nursing Team to develop policy and procedures for medication management, procurement, transportation, storage and dispensing in FNPCI service delivery sites
Supports, advocates for and provides education on use of Traditional Medicines as part of clients' overall wellness plan
Follow, maintain and reinforce medication use policies and processes to ensure proper and safe dosing, storage, handling, dispensing, and/or administration of medications and compounded sterile products
Participates in process improvements to ensure compliance with adverse drug reaction and medication error reporting programs, implements safety mechanisms, and continually assesses patient safety during transitions of care
Participates in patient case management as part of the interdisciplinary care team
Job Specifications
Occupational Certification and requirements
Current registration as a pharmacist with the College of Pharmacists of British Columbia.
Current valid Class 5 B.C. driver's license and willingness to submit a drivers abstract.
Vulnerable Sector Check will be required.
Experience
Three years recent, relevant patient care experience, ideally in a team-based primary care setting or equivalent combination of education, training and experience.
Experience working with First Nations people and communities, providing pharmacy services which integrate holistic Indigenous approaches is an asset.
Care coordination experience within a multi-disciplinary team and including external partners is an asset.
Knowledge
Knowledge and understanding of cultural humility and the history of colonization and its impact on health and wellness of Indigenous people and communities
An understanding of rural and remote communities and their inherent challenges and barriers.
Knowledge of the pharmacology and therapeutic classification of drugs.
Knowledge/awareness of traditional medicines an asset.
Knowledge of the therapeutic management of various diseases.
Demonstrated ability to complete initial and ongoing patient assessments (clinical and diagnostic reasoning) through appropriate/prescribed technical, therapeutic, safety type interventions.
Ability to teach patients and others both one-on-one and in groups.
Ability to integrate best practice and current research evidence to support professional practice decisions and actions.
Knowledge and experience in policy development related to medication management
Experience working in an interdisciplinary team
