We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in optimizing medication therapy for patients, ensuring safe and effective use of medications, and providing expert pharmaceutical care. This position requires strong clinical skills, attention to detail, and the ability to collaborate with healthcare professionals to enhance patient outcomes.

Key Responsibilities:



1. Medication Management:

Review, interpret, and monitor medication prescriptions. Assess the appropriateness of medication regimens for individual patients. Provide recommendations for medication therapy adjustments based on clinical guidelines.
2. Patient Care:

Collaborate with healthcare teams, including physicians, nurses, and other allied health professionals, to develop and implement individualized care plans. Conduct medication reconciliation and develop discharge medication plans. Educate patients and caregivers about medications, including proper usage, side effects, and adherence strategies.
3. Clinical Assessment:

Perform comprehensive medication reviews and patient assessments to identify drug-related problems. Monitor patient outcomes and adjust therapy based on clinical evidence and feedback.
4. Policy and Procedure Development:

Contribute to the development and implementation of pharmacy-related policies and procedures. Engage in quality improvement initiatives related to medication use and safety.
5. Education and Training:

Provide education and training to healthcare professionals on medication management. Participate in continuing education programs to keep up-to-date with the latest advancements
6. Documentation and Reporting:

Maintain accurate and up-to-date documentation of clinical activities and interventions. Report adverse drug reactions and medication errors according to established protocols.

Qualifications:



Education: A Bachelor's degree in Pharmacy (BSc Pharm) or Doctor of Pharmacy (PharmD). Licensure: Current registration with the Alberta College of Pharmacy. Experience: Previous experience in clinical pharmacy, particularly in continuing care or geriatric settings, is preferred. Skills: Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Proficient in the use of electronic health records and pharmacy management systems.

Work Environment:

Typically works in continuing care facilities such as nursing homes, assisted living, or long-term care homes. May involve direct patient interaction and collaboration with multidisciplinary teams.

Job Type: Full-time

Pay: Up to $65.00 per hour
