Position: Community Caseworker (2 positions)

Terms of Employment: Full-time, Contract position for 10 months

Salary: $4,583 per month

Location: Calgary

Job Purpose



To provide holistic, wraparound services to older adults experiencing elder abuse in the community. This includes assessing risk and needs, providing information and support and client advocacy while working towards a safe outcome. The Community Caseworker will work with older adults experiencing abuse who may be previous Shelter clients, community clients or clients referred through the Elder Abuse Resource Line. This role will include opportunities to facilitate presentations to community members and agency partners.

Key Responsibilities:



a. Coordinate with Shelter clients and their caseworkers to initiate and facilitate the discharge planning during case management meetings. The Community Caseworker will connect them with the necessary and applicable referrals and supports needed to be well supported in the community.

The Community Caseworker will conduct regular follow up with clients to complete client-led case plans, work on goals and skill building for 1 year post discharge from the shelter program.

c. Support older adults in the community. This is the "Community Client Caseload." Most of these clients are referred to the community Caseworker through Shelter staff attending to the Elder Abuse Resource Line (EARL).

d. Answer the Elder Abuse Resource Line supporting older adults in the community with referrals, resources, and possible admissions into the Shelter.

e. This role requires you to be knowledgeable about community resources to best support older adult experiencing elder abuse.

f. Plan and deliver activities to regularly engage Shelter residents. The expectation is that the Community Caseworker will build connections with external resources from the community who will contribute their skills/areas of interest with the goal of making the Shelter program as dynamic as possible.

g. Regularly attend and engage in Alberta Elder Abuse Awareness Council meetings, discussing provincial trends of elder abuse, resource sharing, gathering stats and analysis for council reports and supporting case managers in the province with case collaboration and consultation.

h. Facilitate and deliver Elder Abuse Presentations and Alberta Elder Abuse Council materials to community members and agency professionals.

2. Program Administration

a. Ensure that all files are kept up to date. Ensure that every unique contact with clients in the community is properly documented and includes a corresponding summary that details the interaction and includes follow-up action items.

b. Create and maintain Case Plans measuring success of goals and objectives for each client on caseload and if necessary, community clients.

c. Ensure all Community Clients' stats are tracked down and submitted monthly to Shelter Manager. Stats are comprised of ex-Shelter residents, and community clients. Stats should incorporate the number of unique contacts made, the type of contact, the types of abuse present, DOB, and risk assessments, safety planning and support provided to assist clients with domestic abuse/elder abuse.

Keep open communication with the Shelter Manager about Community Client files

3. Emergency Response:

a. Ensure compliance with agency emergency protocols and in the community use all safety training and working alone protocols.

b. Ensure documentation on agency incident forms when emergency situations have occurred whether in the building or in the community.

4. Promotion:

a. Establish and maintain collaborative and professional relationships with community partners, ensuring smooth client referral.

b. Promote awareness of elder abuse issues in the community in conjunction with Shelter Outreach Planning.

c. Work with other Shelters to support and case manage senior clients.

5. Committee Work:

a. Serve on Kerby Centre committees as required or as time and interest permit.

b. Serve on community-based committees as required.

Qualifications:



Education:

Degree in Social Work or other Human Services required. Two-year post-secondary diploma with experience may be considered.

First Aid/CPR AED level C required.

Experience

At least 1 year experience on a crisis line or in a Shelter Familiarity with family violence issues
Preferred. Competencies:

Calm under pressure, organized, good written and verbal communication skills, pro-active and having intermediate level computer skills.

Working Conditions:



This is a fixed-term, full-time position, for 10 months, 8 hours a day including one hour of unpaid break. Shifts are generally, 7:45-3:45 Monday to Friday. May require to flex hours to run programming for clients after hours. Must also be prepared to work extended hours in crisis situations. Must be prepared to work alone and be familiar with working-alone safety protocols.

May be involved in potentially violent or unpredictable circumstances with people who are under emotional stress or mentally ill.
