Position Summary
We are seeking a
Clinical Pharmacist
to join our inter-disciplinary health team. In this role, you will provide
direct patient care
, ensure
safe and effective medication management
, and
support culturally informed wellness practices
, including the integration of Traditional Medicines.
Key Responsibilities
Provide
direct patient care
: assessment, medication review, care planning, follow-up.
Identify and resolve drug therapy problems with patients and the care team.
Develop and implement
medication management policies and procedures
.
Provide
drug information, education, and consultation
to patients and providers.
Participate in
quality improvement/patient safety
(audits, ADR reporting).
Support
holistic wellness
, including Traditional Medicine integration where appropriate.
Build linkages with community pharmacies, health authorities, and service agencies for
continuity of care
.
Maintain confidentiality and uphold professional standards.
Qualifications
Education & Certification
Current registration with the
College of Pharmacists of British Columbia
.
Class 5 BC Driver's License
(driver's abstract required).
Vulnerable Sector Check
prior to employment.
Experience
3+ years
recent, relevant patient care experience (team-based/primary care preferred).
Experience with
First Nations communities
and holistic/Indigenous approaches is an asset.
Policy development
and
care coordination
experience preferred.
Knowledge & Skills
Strong knowledge of
pharmacology
, therapeutic management, and medication safety.
Familiarity with
First Nations Health Benefits
and
PharmaCare
.
Understanding of
rural/remote health
contexts.
Excellent
communication
,
organization
, and
collaboration
skills.
Proficient with
EHRs
and Microsoft Office.
Demonstrated
cultural humility
and awareness of colonization's impact on health.
What We Offer
Salary Range 45.00 - $62.50/hour depending on experience working 70 hours biweekly
