Clinical Nursing Specialist, Paediatric Advanced Care Team (PACT)

Working in collaboration with the interdisciplinary health care team, Clinical Nurse Specialists at SickKids have the opportunity to provide expertise in the areas of:

Clinical Practice Education, Mentorship, and Research Leadership and Organizational & Systems Management




We are currently looking for a'Clinical Nurse Specialist to join our Paediatric Advanced Care Team (PACT), the SickKids palliative care service. As a CNS on the team, you will provide leadership and promote excellence in family-centred nursing care through the five main role components of expert practice, education, consultation, research and leader/change agent. You will care for, and support children, adolescents and families faced with complex, life-threatening conditions across a continuum of inpatient, outpatient and community care.

Here's What You'll Get To Do



Be responsible for the provision of comprehensive care to children with a variety of life-threatening conditions, and their families, in both the inpatient and outpatient setting. Coordinate the interdisciplinary plan of care. Integrate clinical skills and knowledge within a broad scope of advanced nursing practice. Participate in and/or lead complex discharge and transition planning processes. Collaboratively work with community providers and families to deal with challenges as they arise in the community (home and hospice) to help families feel more comfortable remaining at home or facilitate an admission to hospital/hospice. Provide support for the Pathways Grief Support Program. Assume a leadership role in influencing clinical nursing practice and the provision of care. Engage in education and training activities to support nurses across the organization gain knowledge in caring for the specialized needs of these patients. Integrate research into practice. Create, utilize and/or disseminate new knowledge.

Here's What You'll Need





Essential Requirements



Master of Nursing or equivalent with a clinical focus. Registration with the College of Nurses of Ontario 3 - 5 years of clinical paediatric nursing experience. Ability to act as a change agent, and foster teamwork and consensus. Innovative critical thinking with excellent communication, public speaking and problem-solving skills. Knowledge of theoretical frameworks and research application.

Assets



Prior experience as an Advanced Practice Nurse. Experience caring for children with life-threatening conditions and their families.
