Reporting to and working in collaboration with the Senior Director, Organizational Effectiveness and Professional Practice, the Clinical Lead, Interprofessional Practice (MD/NP) provides clinical leadership, champions evidence-informed practices, quality care, clinical excellence and innovation across WellFort through the professional practice model framework. This role is ideal for a WellFort primary care provider passionate about improving client care outcomes, promoting clinical best practices, and engaging collaboratively with clinical peers and community partners.
Accountability:
The Clinical Lead, Interprofessional Practice is accountable for advancing and maintaining clinical practice standards across WellFort's primary care teams. This includes working collaboratively on the development, implementation and evaluation of evidence-based clinical practices and quality improvement initiatives in alignment with the professional practice model. The Lead is responsible for providing professional guidance to clinical staff, supporting interprofessional collaboration, and ensuring adherence to regulatory and organizational policies. Additionally, the lead is accountable for identifying practice gaps, recommending system-level improvements, contributing to policy development, and representing clinical perspectives in internal and external initiatives that impact care delivery.
Major DUTIES & RESPONSIBILITIES:
Clinical Leadership and Practice Excellence:
Serve as a clinical leader and subject matter expert, championing evidence-informed, equity-based and culturally safe clinical practices across clinical teams
Provide clinical mentorship, consultation and case-based guidance to clinicians, supporting sound clinical judgement and high-quality, person-centred care
Collaborate with interdisciplinary teams to develop, implement and evaluate clinical policies, care pathways, and medical directives that strengthen integrated and team-based care
Ensure alignment of clinical practices with regulatory standards, WellFort's professional practice framework and policies
Monitor and interpret emerging clinical evidence, health system trends, and regulatory changes to inform practice development, planning and innovation
Promote health equity, cultural safety and anti-oppressive practices in all aspects of clinical care and leadership
Quality Improvement and System Integration:
Lead and/or co-lead quality improvement initiatives that advance clinical effectiveness, client safety and experience
Identify and address clinical practice gaps, recommending system-level improvements to enhance quality, safety and consistency of care
Contribute clinical insights to organizational planning and external collaborations focused on integrated care, service innovation, and health system transformation
Support the adoption and optimization of digital health systems and tools to improve clinical workflows, communication and data-informed decision making
Lead the annual quality of care chart review process, analyzing findings and developing action plans for learning and continuous improvement
Support monitoring and reporting on key clinical performance indicators and quality metrics to support evidence-based decision making and accountability
Professional Development and Education:
Foster a culture of continuous learning through professional development activities, in-service education and interprofessional mentorship
Support onboarding and orientation of new clinical staff, reinforcing WellFort's professional practice standards, values and interprofessional care principles
Facilitate clinical placements and strengthen academic partnerships through supervision and support of medical learners and family practice residents
Contribute to capacity building initiatives that enhance team skills, leadership development and interprofessional collaboration (e.g. skills simulations)
Stakeholder and Community Engagement:
Represent WellFort's clinical perspective in internal and external committees, working groups and system-level initiatives that impact care delivery
Build and sustain collaborative relationships with community partners, academic institutions and sector organizations to advance shared clinical and health equity goals
Act as a clinical ambassador for WellFort, promoting integration, partnerships and innovation in primary care and community health
Minimum Qualifications and skills:
Experience:
Minimum 3 years' experience in primary care clinical practice
1-3 years' experience in a clinical leadership role
Knowledge:
Demonstrated expertise and strong interest in clinical quality improvement methodologies
Skilled in policy development, documentation, and implementation
Commitment to health equity and a social determinants of health approach within clinical practice and community health settings
Clinical teaching experience and affiliation with Toronto Metropolitan School of Medicine, or willingness to obtain
Experience or interest in community-based research and community academic partnerships
Skills:
Strong analytical and critical thinking skills
Excellent communication, interpersonal and conflict resolution skills, with proven ability to speak, listen and write in a clear, thorough and timely manner using appropriate and effective communication tools and techniques

Education and Certifications::
Licensed physician (MD) or nurse practitioner (NP) in good standing with corresponding regulatory college
Clean driver's abstract, current, valid Class "G" Driver's License and reliable transportation to support travel across WellFort's various sites, as required
Working conditions:
Lifting, carrying, handling of small objects and packages not normally exceeding 1-10 lbs., e.g., files, information brochures, small office equipment/tools.
Sitting for long durations, e.g., meeting with people, driving, working at desk.
Walking and standing for short durations.

As a condition of employment, new WellFort staff must be fully vaccinated unless they have received an exemption from vaccination under the Human Rights Code. Proof of COVID-19 vaccination status will be required before the first day of work or, proof of religious or medical exemption, if or where applicable.
Fully vaccinated is defined as having received the completed series of an accepted COVID-19 vaccine, as recommended by the Office of the Chief Medical Officer of Health and having received the final dose at least 14 days before your employment start date.

Acceptable proof is a Ministry of Health Dose Administration Receipt (or such other proof of vaccination that the Province of Ontario sanctions).
