Reporting to the Chief Operating Officer of Long-Term Care (COO-LTC), the Clinical Excellence Lead is a strategic clinical leader responsible for advancing best practices, introducing new programs, and supporting clinical teams in delivering exceptional resident care. This position will guide the development of policies, standards, and professional practices that align with our Mission, Vision, and Values--while ensuring compliance with the Fixing Long-Term Care Act (FLTCA) and accreditation requirements.
The Lead will collaborate with site leadership and administrators to introduce and sustain innovative clinical programs, foster interprofessional collaboration, and champion a culture of continuous improvement. This role will also ensure that clinical initiatives are implemented in a way that supports long-term sustainability and does not overwhelm site capacity.
Key Responsibilities
Champion the integration of evidence-based practices into clinical programs, standards, and policies.
Develop and maintain organizational policies and protocols to ensure consistency and quality across all long-term care sites.
Mentor and support site clinical leaders, promoting professional development and evidence-informed approaches to care.
Lead change management efforts for new program rollouts, ensuring site readiness and long-term sustainability.
Oversee compliance with provincial legislation and accreditation standards, including Infection Control policies.
Engage residents, families, staff, and community partners in the design and evaluation of programs.
Monitor outcomes, track progress, and prepare reports to inform ongoing planning and decision-making.
Qualifications & Competencies
Current certificate of competence from the College of Nurses of Ontario; BScN preferred.
3-5 years of experience in geriatrics, rehabilitation, or long-term care nursing.
Strong knowledge of the Fixing Long-Term Care Act, 2021 and related regulations.
Proven ability to lead clinical teams and implement evidence-based improvements.
Expertise in change management, policy development, and program evaluation.
Strong leadership, coaching, problem-solving, and analytical skills.
Proficiency in Microsoft Office and digital documentation tools.
Flexibility to travel between LTC operational sites.
