This position requires an active and current nursing license. You would work closely with CPCN and management to ensure quality improvement goals are met, as well as with patients, creating a patient medical home motto.
Duties
Support and/or lead implementation of the CPCN charter
Assists in establishing and maintaining performance measurements and monitoring principles in population based care within CPCN
Utilize principles of AIM and improvement to enhance access, office efficiency, process and clinical care
Provide bi-annual updates to CPCN and Quality Improvement lead
Maintain compliance with healthcare regulations by accurately documenting procedures and safeguarding patient confidentiality.
Manage or assist in management of physician panels. Proactive screening, test/blood work completed prior to physician visit.
Develop programs or processes which support panel population health
Skills
Proven experience working in a medical office environment with strong familiarity of medical terminology.
Excellent organizational skills with the ability to multitask efficiently in a busy clinical setting.
Strong communication skills to effectively interact with patients, healthcare providers, and administrative staff.
Office experience that demonstrates proficiency with standard computer software and healthcare management systems.
Prior clinical office experience is a plus, highlighting versatility in various clinical environments.
Ability to adapt quickly to new procedures and maintain professionalism under pressure. The CCC may be asked to assist clinic manager with supervision, mentorship of team members.
