As a Client Care Worker in the Inner-City Health Program, you will be responsible for providing personal care, medication assistance, and support to clients in specialized units. This is a full-time position to work EVENINGS. Carefor is committed to providing quality care to its clients twenty-four (24) hours a day, seven (7) days a week. In order to accomplish this, you will be expected to work the schedule as posted. However, Carefor does reserve the right to change the schedule as required.

What You Will Do:

Acts within the unregulated worker's role when making observations regarding the client's health status, condition and situation Uses information in the care plan, computer memos, staff communication log, and the electronic chart when making observations, and seeks out guidance and information from the nurse, supervisor or other relevant OICH staff members as required Reports all changes in client condition/situation in accordance with the Unit policies and procedures Contributes to the development and revision of service plans in collaboration with the Unit nurse Sets service goals in collaboration with the client and the Unit nurse and other members of the multidisciplinary care teams, including medical staff, support staff, location managers and members of outside partnerships
Qualifications:

Personal Support Worker or Developmental Service Worker certificate Non-Violent Crisis Intervention training or willing to be trained Suicide ASIST training or willing to be trained Addictions training or willing to be trained Motivational Interviewing or willing to be trained Current CPR and First Aid Experience providing personal care mandatory Experience in Palliative Care an asset Experience working with the homeless population an asset Experience working with clients who have mental health diagnosis/challenges and/or are substance abusers an asset. Experience administering and documenting medications an asset Bilingualism an asset Demonstrated ability to form effective helping relationships Comfortable working within the Harm Reduction Philosophy of care and administering opioids to clients under the safer supply concept of care Willingness to complete additional training as required, including formal workshops and in-services and individualized teaching Ability to recognize own strengths and limitations and to seek guidance when necessary Ability to work effectively in a multidisciplinary team Excellent observation skills Basic computer keyboarding skills, documenting on a handheld device, online learning Willingness to respond positively to supervisor feedback Ability to work independently with minimal supervision Demonstrated ability to work with challenging clients Problem-solving and decision-making skills Ability to recognize and respect the cultural, religious and spiritual beliefs of clients. Ability to respect the client's right to safety, dignity, privacy, and respect at all times
