We're a non-profit, team-based primary care clinic dedicating to helping clients maintain and improve their health. Our integrated services including a medical clinic, dental care, pharmacy and parent support program, ensure that people get the care they need, all under one roof. The Opportunity

We're looking for a vibrant and dynamic person to join our team as a Client Care Coordinator (C3). The C3 is a vital member of the interprofessional healthcare team, serving as the first point of contact for clients and ensuring seamless clinic operations. This role goes beyond traditional administrative tasks, its about:

• Creating a welcoming and inclusive environment
• Optimizing scheduling and workflow efficiency
• Strengthening communication between clients, providers, and the broader care team.

The C3 plays a key role in enhancing the client experience, supporting team collaboration, and contributing to the overall effectiveness of the clinic.

Key responsibilities:

Be the welcoming face and voice of Mid-Main--ensuring every client interaction is warm, respectful, and person-centered.

Support clients in navigating healthcare services, coordinating appointments, referrals, and follow-ups.

Track and coordinate chronic disease management follow-ups, preventative screenings, and ongoing care plans to ensure clients stay on top of their health needs.

Proactively identify and remove barriers to care, connecting clients with the right supports.

Foster clear communication between clients and providers, ensuring timely follow-ups and seamless coordination.

Manage and optimize provider schedules to keep the clinic running smoothly.

Act as the key link between clinical and administrative teams, facilitating smooth communication and problem-solving.

Onboard new team members and integrate them into clinic workflows.

Participate in team huddles and quality improvement initiatives to enhance efficiency and patient care.

Maintain accurate electronic medical records (EMR), scheduling, and documentation with high attention to detail.

Support chronic disease management tracking by ensuring follow-ups, recalls and preventative screens (e.g., diabetes care, hypertension monitoring, cancer screenings) are completed and documented.

Process referrals, medical documents, and billing with efficiency and accuracy.

Support clinic safety, infection control and wellness efforts.

Monitor client outcomes and clinic workflows to identify opportunities for improvement, streamline processes, and enhance care delivery.

Contribute to a culture of continuous learning and improvement by bringing forward insights, participating in team discussions, and supporting quality improvement initiatives.

What You Bring to the Table:

Previous experience in healthcare administration, primary care, or community health.

Strong interpersonal and communication skills--you build relationships effortlessly.

Tech-savvy--comfortable with EMR systems (particularly OSCAR) and scheduling tools (an asset).

Ability to work in a fast-paced team environment.

A problem-solver with the ability to think on your feet and adapt in a dynamic clinic setting.

High level of organization, attention to detail, and professionalism.
