The MLTC Health & Social Development Department is seeking a qualified Licensed Practical Nurse (LPN) to fill the full-time, permanent position of Chronic Disease Management Support Case Manager.This Position plays an essential role in two components of MLTC Health Services Delivery Plan. Under the Direction of the MLTC Nursing Manager, The MLTC CDM support Case Manager will be expected to work with each of the nine Meadow Lake First Nations to continue to achieve their vision for Chronic Disease Management that includes assisting the team lead Nurse Practitioner who will run the CDM clinic and follow up and assist patients in the communities for various programs and plans of care prescribed for them by the NP. This position will also integrate in-depth clinical expertise and experience in the provision of diabetes prevention and education, early screening of patients who are at risk for complications such as kidney failure, hypertension, eye problems and neurological problems etc. The case manager will be supervising the client's dietary intake, drug therapy and ensure they follow the self-management program. In addition to clinical care, they will also provide all necessary education and preventive measures to help the client control diabetes and live a quality life.MLTC offers competitive wage comparison. Program Reporting Requirements Communication, Advocacy, and Promotion of Independent Living Assist in procuring supply and equipment resource for patient with Chronic diseases admitted to your program Program Education Advocate or coordinate with/for Client/Families; Health Professionals; Students and Community Organizations Booking appointments, taking directions from the CDM Nurse Practitioner, continue working with Electronic Medical Records (EMR) to follow up task related referrals, lab work and follow up results and ensure the data is available in the patient folder Follow up with clients for no-shows with appointments. Motivate clients to attend clinic and programs.QUALIFICATIONS/SKILLS/ABILITIES: Minimum: Licensed Practical Nursing Diploma; registered or eligible for registration with SALPN Familiar with Electronic Medical Records (EMR). Will possess excellent interpersonal, time-management, and organizational skills; excellent communication (written/verbal) and presentation skills. Preferred experience in Nursing full to include some of the following: clinical experience in Chronic Disease & management; Community Nursing; Client/Family health education, facilitating, and leading group activities; program/resource development related to needs assessment, planning, program design, implementation, and evaluation; experience working with Indigenous people. Proficient with computers, i.e., Microsoft Office, Word, Excel, Power point and Outlook. Ability to travel extensively and possession of a valid driver's license.Preferred: Chronic Disease Management in particular diabetic education / certification or a willingness to obtain and maintain this certification. (MLTC will support obtaining this certification) Knowledge and understanding of First Nations and Metis customs, family systems, practices, health needs and health care organizations.
