About the Opportunity

The Hants Health and Wellness Team (HHWT) is a community based, integrated chronic disease prevention and management service that provides wellness programming and navigation, chronic disease management, nutrition education, emotional wellness counselling and complex care. The HHWT is an interdisciplinary team that builds community connections and partnerships to provide programs and services that are responsive to community needs. Principles of patient-centred care, behaviour change and self-management are core to all programming and services.

Reporting to the Health Services Manager, Primary Health Care, the incumbent will use critical thinking and clinical judgment supporting patients (in both individual and group-based settings) with the management of their chronic disease, with a particular focus on diabetes, hypertension and dyslipidemia. This includes medication management, insulin initiation, insulin adjustment and follow up, diabetes nutrition/physical activity counseling, self-management support and providing/facilitating links with resources in the community. As a member of an interdisciplinary team the successful candidate is outcome-focused, incorporates evidence based/best practice, and emphasizes health promotion and disease prevention.

About You

  • Bachelor of Science in Human Nutrition
  • Completion of Dietetic Internship accredited by Dietitians of Canada
  • Licensure by Nova Scotia Dietetic Association required
  • One year of related experience required working in a Primary Health Care setting/community-based team/community setting and experience with people living with a chronic condition; with individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principles
  • Certified Diabetes Educator Certificate (CDE) required (or eligible within one year of hire)
  • Willing to complete requirements for Delegated Medical Function
  • Preferred experience in partnership development; stakeholder collaboration, and community development
  • Understanding of population health, health promotion strategies, and behaviour change principles
  • In-depth knowledge of local Nova Scotia Health and community-based services
  • Demonstrates person-centred / family-focused, diversity relevant approaches to care
  • Well-developed interpersonal, teamwork, oral and written communication skills
  • Demonstrates flexibility, adaptability and willingness to change
  • Demonstrates innovation in program planning, implementation and evaluation, and quality improvement, planning skills
  • Computer skills (MS Office, email and internet)
  • Annual CPR-AED certification and annual WHMIS training and certification
  • Valid Nova Scotia Driver's License and access to a reliable car for travel for work
  • Required to work at multiple sites and travel between various community locations throughout the Zone
  • Competencies in other languages preferred; French an asset

Compensation and Incentives

$31.83 - $41.95 hourly
