We are seeking a dedicated Nurse Practitioner to join our healthcare team. The ideal candidate will have experience in providing primary care, conducting assessments, and developing treatment plans. Due to the demographics of our patients and our location being in Chinatown, being able to speak Mandarin Chinese or Cantonese is a strong asset.
Responsibilities
Conduct comprehensive patient assessments and physical examinations
Develop and implement individualized treatment plans based on patient needs and medical history, including prescribing medication.
Collaborate with multidisciplinary teams to coordinate patient care and case management.
Utilize electronic health records (EHR) systems such as Accuro
Educate patients and families on health management, disease prevention, and wellness strategies.
Requirements
Current Nurse Practitioner certification
Mandarin-speaking skills is preferred
Experience providing primary care is preferred.
Familiarity with case management practices is a plus.
Strong understanding of medical terminology relevant to diverse patient populations.
Excellent communication skills
Compassionate approach towards patient care with a focus on promoting health and well-being.
