We are seeking a dedicated and compassionate
Continuing Care Assistant
to join our team. The ideal candidate will have experience working with individuals across the lifespan and possess a strong understanding of various mental health and addiction concerns. This role involves providing psychosocial assessments, developing treatment plans, and delivering therapeutic interventions to support the mental health and well-being of our clients.
Responsibilities
Conduct psychosocial assessments and evaluations for clients experiencing a range of mental health and addiction issues.
Develop individualized treatment plans based on assessment findings.
Provide evidence-based therapeutic interventions tailored to the needs of clients.
Collaborate with multidisciplinary teams to ensure comprehensive client care.
Maintain accurate and detailed records of client progress and treatment outcomes.
Monitoring medications for paitents and checking vital's when applic.
Utilize culturally sensitive approaches.
Engage in crisis intervention and provide support when necessary.
Qualifications
CCA certification (or equivalent recognized training)
Experience in mental health, long-term care, or wellness settings is an asset
Strong interpersonal skills with a calm, compassionate approach
Ability to manage challenging behaviors with patience and professionalism
Comfortable working in a team-based, client-focused environment
CPR/First Aid certification (or willingness to obtain)
Ability to work collaboratively within a multidisciplinary team.
Familiarity with issues such as depression, anxiety, PTSD, addiction, and other mental health conditions.
Strong organizational skills.
Experience working with diverse populations in clinical settings is considered an asset.
Experience working with Indigenous communities or knowledge of Indigenous cultures is considered an asset.
Job Types: Full-time, Part-time
Pay: $20.00-$25.00 per hour
