Position Summary
We are seeking a skilled and dedicated
Case Manager
to join our team. This role is responsible for coordinating and delivering comprehensive case management services to youth, supporting them across multiple life domains including education, employment, mental health, housing, cultural identity, and life skills. The Case Manager will work closely with youth and their support networks to develop individualized plans that promote stability, well-being, and long-term success.
Key Responsibilities
Provide direct case management, including intake, assessment, care planning, support, and referrals across various domains such as mental health, education, housing, cultural/spiritual development, financial literacy and other life skills.
Facilitate youth-centered support teams and individualized support plans using strength-based and resilience-focused approaches (e.g., Circle of Courage, wraparound).
Collaborate with internal team members and external partners to ensure coordinated, consistent, and meaningful supports.
Coordinate, facilitate, and deliver engaging and relevant programming including cultural activities, life skills development, career exploration and training or certification opportunities.
Maintain accurate and timely case notes, reports, and documentation in accordance with policies and best practices.
Facilitate regular case conferences and communicate effectively with natural and professional support networks.
Support administrative, financial and operational functions as needed.
Qualifications & Requirements
Post-secondary education in Social Work, or social services (i.e. Psychology, Sociology) or a related field.
Strong understanding of adolescent development, trauma-informed practices, and resilience models such as the Circle of Courage.
Familiarity with local community resources and experience working within mandated systems (e.g., child and family services, justice, education).
Excellent written and verbal communication skills.
Strong organizational and time management skills, with the ability to prioritize and work independently or collaboratively.
Flexibility to work varied hours, including some evenings as occasionally required.
Valid driver's license and access to a vehicle for work-related travel.
Clear Criminal Record Check and Child Abuse Registry Check (current and valid).
