Shepherds of Good Hope (SGH) is one of the largest not-for-profit organizations dedicated to meeting the needs of individuals experiencing homelessness and vulnerably housed individuals in the city of Ottawa. Many individuals who access our programs and services live with complex trauma, mental health challenges and substance use disorders.
A leader in supportive housing and harm-reduction, you'll be joining an organization that provides around-the-clock, comprehensive services to all genders in seven supportive housing, shelter and programming locations across the city. RESPONSIBILITIES
In collaboration with residents, staff and external workers, develop care plans and provide practical assistance
Providing support related to housing, employment, financial aid, health care, social supports, and other areas as identified
Supporting guests in building life skills; developing and facilitating weekly life skills and recreation activities service users
Develop strong relationships with both current and new partner agencies
Broker linkages to relevant community supports for guests
Completing SPDAT, OCAN, OPOC, GAIN S.S. assessments and other assessments, as needed
Submit accurate and complete records and data reports, as required
Work collaboratively with partnering agencies and colleagues
Engage in guest advocacy
Provide frontline support and perform additional duties as required
Assist with completing service user yearly taxes
Assist with transportation and accompanying service users to appointments, as required.
Assist with intake process for new residents.
QUALIFICATIONS
College Diploma with at least two years of relevant experience; Bachelor's Degree preferred; Postgraduate Degree would be considered an asset
Experience working with, and knowledge of, resources concerning homeless populations, addictions, mental health, housing, and poverty issues
Strong written and verbal communication skills
Problem solving and organizational skills
Fluently bilingual in French and English considered an asset.
Strong interpersonal skills and ability to collaborate
Computer proficiency
Demonstrated ability to build rapport and work within the Harm Reduction framework, using NVCI principles.
Knowledge of case management principles and tools
Demonstrated knowledge of Landlord and Tenant rights and responsibilities
Access to a personal vehicle to utilize to get to the various Supportive Living buildings is preferred* A valid Class G, Ontario driver's license and clean driving record is preferred* Must be legally entitled to work in Canada
Must by at least 18 years of age
Able to lift up to 25kg.
Able to read text and communicate orally in English. The ability to communicate in other languages is considered an asset.
Working Conditions:
There is a combination of long periods of sitting, standing, and walking for this position. This role is mostly consumed with client engagement, which can mean meeting in private and confidential locations with clients on a one-to-one basis. Accuracy and timeliness are very important to this role to ensure that client needs are being met and reporting is complete. The case manager must also be comfortable utilizing critical thinking to make well rounded decisions in high pressure situations and prioritizing work based on competing deadlines.
