What We Will Ask of You:



As a key member of our Adult Urgent Response Services (AUR) team, our Case Manager plays an important role. In collaboration with the AUR team, the Case Manager ensures provincial program goals and principles are adhered to and services are provided in an efficient and effective manner and continue to be in line with best practice for quality service provisions. On most days you'll be providing short-term services to adults who live with a developmental disability, mental health need, challenging behaviour, and/or involved or at risk of involvement within the criminal justice system as deemed eligible by Developmental Services Ontario (DSO). This position also involves housing advocacy, direct support in housing search efforts, and collaboration with local agencies to connect clients with essential services both within and outside of the developmental services sector. The Case Manager is a champion of Canopy's core value of Caring Community who ensures individuals experiencing crisis are cared for, this may include guiding clients in exploring housing options and developing individualized action plans for securing sustainable housing.

Primary Responsibilities:



Provide short-term case management and customized supports to assist adults living with a developmental disability and/or mental health need who are experiencing crisis as deemed eligible through MCCSS. Conduct individual risk assessment to develop an Individual Support Plan (ISP) including applicable risk factors, safety protocols and other customized supports as required. Monitor and evaluate service user progress within ISP and ensure individual's goals are achieving desirable outcomes. Complete funding requests for individuals, and in collaboration with Team Lead, manage approved funds. Attend applicable appointments, case conferences and other supportive meetings as required. Provide guidance and expertise of sector resources to individuals navigating service options including; Adult Service Coordination Committee, Developmental Services Ontario (DSO) and Canopy. In conjunction with the Program Manager and Team Lead, provide ongoing support and collaboration in developing community outreach materials including; communicating information sessions/events with external stakeholders, preparing outreach materials, and supporting with other program related activities/community initiatives. Maintain ongoing documentation as required within agency policy and procedure manual including consents, progress updates, service agreements, crisis plans, safety plans, risk assessments as well as other applicable service user information in Client Database Manager (CDM).

Qualifications:



Completion of Bachelor's or Master's degree in Social Sciences or related field or equivalent work experience. 3 years of experience in Case Management or direct support experience in a residential or community setting. Case Management or mental health experience is considered an asset. Direct experience working with people with an intellectual disability, dual diagnosis or mental health need or equivalent. Understanding of relevant legislation including; Ontario Human Rights Code, Accessibility for Ontarians with Disabilities Act, Occupational Health and Safety Act, Ministry of Children, Community and Social Services (MCCSS), Personal Health Information Protection Act (PHIPA), Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act and Quality Assurance Measures (QAM), and its application. Working knowledge of DSO and MCCSS's Serious Occurrence reporting guidelines. Valid G-Class driver's license for the Province of Ontario, access to reliable transportation and have valid proof of automobile insurance with driving for business purposes and a minimum liability of two million dollars.
