The Canadian Mental Health Association (CMHA) - Toronto is looking for a

Case Manager

to join our

Mental Health and Justice Prevention Program

. This role supports individuals living with serious mental illness who are at significant risk of criminal charges or police involvement. Working as part of a team serving East Toronto, the Case Manager will provide community-based supports, promote recovery, and foster stability through comprehensive case management and advocacy.

Salary:

$54,943 - 60,203

Hours of Work:

35 per week

Responsibilities

Conduct outreach and initial assessments Provide orientation to the program and agency Engage clients using a client-centered, strengths-based approach Develop individualized service plans and support skill-building Connect clients with resources, information, and advocacy Build and maintain trusting, respectful, and collaborative relationships Monitor progress, evaluate outcomes, and conduct regular follow-ups

Experience & Education Required

Bachelor's degree in Social Work, Social Sciences, or equivalent experience Minimum of 3 years of experience in community mental health and/or addictions Proven experience in case management Demonstrated ability to work with diverse and marginalized populations Commitment to anti-racism, anti-oppression, and 2SLGBTQIA+ affirming practices Proficiency in a second language is an asset Valid driver's license, access to a vehicle, and appropriate insurance coverage
