Salary Range: $58,662 - $64,101

We provide accessible, quality, culturally safe and holistic health care that supports and enhances the wellness of individuals, families, and communities. We work with our 11 First Nation community partners and the IFC in the provision of primary health care, traditional healing, mental wellness and addictions, home and community support services, and health promotion services.

The opportunity:

Under the supervision of the Regional Manager, the Case Manager is responsible for providing culturally grounded, trauma-informed, and relational case management support services to Aboriginal adults (18+ years) living with mental health, substance use, and/or homelessness concerns. This role's key function will be to assess clients' needs, develop and monitor personalized wellness plans, to assist clients' in achieving goals set out in their wellness plans while strengthening their connection to identity, culture, and community supports. The Case Manager works collaboratively with clients, their families, Elders, Knowledge Keepers, community partners, and internal program teams to promote wellness, stabilization, and long-term recovery.

What you need to bring with you:

Preference will be given to those individuals with a Bachelor of Social Work (or related degree) and 2 years related work experience (or equivalent combination of education and lived experience). Experience working with Indigenous populations, mental health and addictions, trauma-informed care, and case management practices. Knowledge of local Indigenous communities, culture, teachings, language, and traditional health approaches (or willingness to learn). Demonstrated critical thinking and problem-solving skills. Demonstrated ability to build rapport and clearly communicate with clients. Ability to work independently and collaboratively as part of a multidisciplinary team. Proficient skills in computer programs and electronic case management systems. Knowledge and experience of Anishinabek Health and Traditional healing practices. Excellent oral and written communication skills. Demonstrated discretion and maturity when handling confidential information. Successful vulnerable sector check is required. Ability to work flexible hours; and ability to travel across catchment area. Must have access to a personal vehicle and willing to travel regularly within the Maamwesying catchment area. Must have a valid G driver's license.

What to expect?

Given the traditional practices of Indigenous people, from time-to-time exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage or cedar may occur.
