McMan is recruiting for a

Case Manager

to join our team providing a housing first and harm reduction approach; the Case Manager will be responsible for assisting residents in maintaining their housing, assist accessing financial supports, supporting their education/employment goals and any other areas of support that is required to help them successfully transition into independence. The Case Manager will work alongside our Transition Specialists, by being a member of the resident's team that is planning around the needs for their successful transition into the community.

This role requires flexibility in work hours together with evenings and weekends. This position is on a rotating schedule of Monday-Friday 8am-4pm and Monday-Friday 12pm-8pm. This role will be required to provide on-call support on a rotating basis for the Agency.*

Some of the key aspects of this role include:



Educating and supporting individuals to increase skills that promote greater self-efficacy. Providing individual instruction, modeling, and support of basic life skills (ie: health, nutrition, home cleanliness, safety) Monitoring and supporting individuals through a harm reduction approach to ensure living conditions are safe and sanitary. Implementing integrated service delivery from the point of referral to program completion and transition for individuals accessing the program. Completing case management responsibilities that include assessments, goal planning, case consultation, monitoring and evaluation of the goals to meet the client's needs.

The successful candidate will have:



A degree or diploma in Human Services Three (3) years' experience working with vulnerable populations Strong writing, organizational, program planning and interpersonal skills Understanding of Rapid Rehousing Demonstrated interest and knowledge of the Homeless Serving System of Care. Experience with HMIS data preferred Current standard First Aid Certification and Suicide Intervention Training Valid Driver's license and adequate insurance coverage. The successful candidate must also have a Child Welfare Intervention Check and Police Check (current within 6 months)

Salary Range: $49,768.00 - $54,931.00 annually
