Case Manager

to join our

Work It Forward

program. In this role, you will work alongside Occupational Therapists and Job Developers to help clients build confidence, develop job readiness, and progress toward successful employment.

About the Role



As a Case Manager, you will guide clients through all four phases of the program, combining one-on-one coaching with group facilitation. Using a trauma-informed and strengths-based approach, you will help clients identify their goals, create personalized action plans, and access the tools and supports needed to succeed. You'll play a pivotal role in keeping clients engaged, ensuring accessibility, and tracking progress in alignment with the clinical insights of our Occupational Therapy team.

Responsibilities



Conduct intake, orientation, and needs assessments, supporting the OT during evaluations. Facilitate career exploration and co-develop individualized action plans. Support clients in simulated work activities, monitoring progress and providing coaching. Deliver workshops on job skills, self-regulation, stress management, and workplace communication. Provide ongoing one-on-one progress meetings, employment readiness coaching, and transition support. Assist with onboarding and workplace integration during employment placements. Maintain accurate client records and contribute to program reporting. Collaborate with internal team members, community partners, and external supports to ensure wraparound service coordination.

Qualifications



Required:



Diploma or degree in Social Work, Psychology, Disability Studies, Human Services, or related field. 2+ years' experience in case management, ideally in employment, disability services, or mental health. Strong knowledge of trauma-informed, strengths-based approaches. Experience supporting clients with cognitive, psychosocial, or neurodevelopmental barriers. Excellent interpersonal, facilitation, and documentation skills. Proficiency in Microsoft Office and comfort with case management systems.

Preferred:



Experience in interdisciplinary or OT-led service delivery. Familiarity with assessment tools (e.g., MOHOST, DOA) - training available. Group facilitation experience with diverse learners. Knowledge of inclusive employment practices and the Alberta labour market. Training in motivational interviewing, conflict de-escalation, or mental health first aid.
Job Types: Full-time, Permanent

Pay: $55,000.00-$62,000.00 per year
