Compensation: Salary based on Experience + Benefits following a successful probation

Type of Employment - Full time (37.5 hours/week) (Subject to NS Works Contract Agreement)

Reporting to the Operations Manager, the

Case Manager

is responsible for employability services for residents of Cumberland County and will work directly with clients and the business community and other stakeholders, promoting employment for all citizens facing barriers to employment. Working within a client-centered model, the Case Manager will provide the necessary interventions to ensure seamless service between staff towards a successful job securement and subsequent follow-up as deemed appropriate by Employment Nova Scotia. They will provide seamless service, from intake/assessment to successful job placement and subsequent follow-up with clients.

Responsibilities:

Follow the Case Management guidelines outlined in the Objectives and Description of Activities provided on the NSEAS Service Agreement

Explain and review of all programs and services, discuss expectations and processes

Promote a positive understanding of employability issues for citizens facing barriers.

Work closely with counsellor to enhance employability skills and document all pertinent information and keep Operations manager informed of issues

Report on case managed clients using LaMPSS

Maintain detailed and accurate records of all client interactions (Keep electronic files up to date)

Follow - ups including 24 & 52 week after closing of action plan

Make appropriate referrals as required (Job Developer, Job Coach. Employment Maintenance worker, Mental Health, CANU)

Keep current on Labor Market Information

Contact and converse with partners as required on behalf of clients' needs

Adhere to a strict confidentiality / non-disclosure code of ethic at all times

Attend meetings and events as required

Foster a work environment that is collaborative, co-operative, and respectful

Other duties as assigned by Operations Manager or Executive Director

Establish and maintain relationship with organizations and individuals who support the persons with barriers to employment

Other duties as assigned by Operations Manager/Executive Director



may include but not limited to:

Fill in at front desk as required

Community participation: attend meetings, events, & job fairs

Prepare and facilitate workshops

External presentations for employment opportunities and promotions of services

Professional Development - life-long learning

Keep current on Labour Market Information and Employment Nova Scotia programs / policies, as well as community resources

Accountability: Employment Nova Scotia Targets:

1. Case Manager will document Intake and Needs Determination

2. Case Manager will complete Return to Work Action Plans

3. Case Manager will complete Skill Enhancement process with clients returning to school or other training including SEB

4. Case Manager will refer job ready clients to Job Developer for employment and or sponsorship (JCP, TWS other approved opportunities)

5. Case Manager to refer clients to Counsellor for accommodation assessment, psych-ed assessments and other career-related assessments

Qualifications:

Degree or diploma, certificate in Human Services Program, Counselling Certification and or courses relevant to the position

Experience working with persons facing barriers in a supportive role

Certified Career practitioner Designation (CCDP) or willingness to obtain within 3 years

Superior communication and presentation skills

Problem solving ability

Knowledge of labor market issues and information

Knowledge of resources and programs available to the community

Knowledge of LaMPSS

Criminal Record and vulnerable persons check

Proficiency in Microsoft Office suite programs
