ABOUT THE ROLE
Under the direction of the
Director of Programs and Indigenous Engagement,
the
Case Management Coordinator
is responsible for overseeing
person-directed care planning
for Community Space participants.This role plays a critical part in addressing the challenges of
houselessness and poverty
in Edmonton by enhancing coordinated
service navigation and case management.
Bissell Centre is transforming
Bissell West
into a
Community Access Hub,
a centralized and low-barrier entry point where individuals can access Bissell's programs, intake services, and referrals to partner organizations.The Case Management Coordinator ensures
seamless pathways
to essential services such as
housing support, mental health services, and financial empowerment
, helping to remove barriers to service access.
This role requires
strong relationships
with community organizations and internal programs to support individuals in achieving their goals. Additionally, the Coordinator will work closely with
frontline staff and program supervisors
to foster collaboration and ensure positive outcomes for participants.
WHAT IS THE ROLE
Duties and Responsibilities
Program Delivery
Collaborate with various programs to develop individualized care plans for community members.
Support programs providing ongoing case management and service coordination.
Conduct intakes and discharges using the Orbit case management system.
Perform needs assessments, referrals, and advocacy in coordination with internal teams.
Conduct comprehensive psychosocial assessments as needed.
Assist programs in assessing and enhancing participants' readiness for positive changes related to mental health and substance use.
Coordinate and deliver group programming to strengthen daily living skills and reduce isolation and loneliness.
Work with case management and frontline staff to ensure participants' care goals are being met.
Facilitate access to external support services, including physicians, agency workers, AHS staff, and home care providers.
Identify and implement outreach stabilization activities, including connections to community agencies and social services.
Collaborate with the Community Engagement Liaison to develop partnerships that enhance service offerings.
Maintain updated information on partner programs and agencies.
Attend case conferences, interagency meetings, and ongoing training to stay informed about best practices.
Provide crisis support and intervention when needed.
Coordinate with programs to make referrals for AISH, ID clinics, home care, PDD, and other support programs.
Administrative
Maintain accurate case notes, program records, and critical incident reports within established timelines.
Track and report program statistics and outcomes as required.
Submit mileage and expense logs for reimbursement.
Participate in annual performance reviews and supervision meetings
WHAT YOU BRING TO THE ROLE
Qualifications & Commitment to Values
Formal Education and Experience
Degree or diploma in Human Services or related field.
2-3 years direct experience working with vulnerable populations.
Experience supporting individuals experiencing homelessness is considered an asset.
Knowledge of Indigenous worldviews, cultures, and the Truth and Reconciliations Commission's Calls to Action is also beneficial.
Knowledge, Skills & Abilities
Strong written, verbal, and interpersonal communication skills.
Solution-focused approach with excellent time management, organizational, problem-solving, and advocacy skills.
Proficiency in Microsoft Word and Excel.
Familiarity with issues related to poverty, homelessness, and Edmonton's urban core.
Experience working with high-risk populations and an understanding of Indigenous cultures, history, and contemporary issues.
Proven ability to provide individual and group support services, develop programming, and facilitate workshops.
Working Conditions
Flexibility of the working week is inherent in this position as the workload fluctuates. Flexibility to work a combination of weekdays and weekends is required.
This position requires participation in organization wide Community Safety Walks in the area of Bissell East and West and will involve daily interaction with the participants and community members served.
